Hemorrhage
Hello, Doctor: I am six months postpartum and had an episiotomy.
I had a Pap smear 50 days after giving birth, and since then, I have experienced a few episodes of bleeding.
The earlier episodes had a similar volume to the bleeding after the Pap smear and were somewhat painful.
Recently, I have noticed some blood-tinged discharge, but there is no significant pain.
I am unsure if this is normal (I am still breastfeeding and have not had a menstrual period).

Hello: If both the Pap smear and the obstetric and gynecological ultrasound examinations are normal, there is no need to worry.

Postpartum bleeding and other related symptoms can be a source of concern for many new mothers. It’s important to understand the normal physiological changes that occur after childbirth, as well as when to seek medical attention.
In your case, having given birth six months ago and experiencing some bleeding and discharge is not uncommon. After delivery, women typically experience a discharge known as lochia, which consists of blood, mucus, and uterine tissue. This discharge can change in color and consistency over time, starting from bright red (lochia rubra) to pinkish-brown (lochia serosa) and eventually to a yellowish-white (lochia alba). The transition through these stages can take several weeks, and it is normal for the discharge to fluctuate in volume and color, especially if you are engaging in physical activity or if your body is adjusting to hormonal changes.
The fact that you have had a few instances of bleeding after your Pap smear and that the bleeding was similar in volume to what you experienced post-delivery is something to monitor. It is not unusual for some women to experience light bleeding or spotting after a Pap test, as the cervix can be sensitive after childbirth. However, if the bleeding is accompanied by significant pain or if it becomes heavier, it would be prudent to consult your healthcare provider.
Regarding the blood-tinged discharge you are currently experiencing, it is worth noting that this can be a normal part of the postpartum healing process, especially if you are still breastfeeding. Breastfeeding can influence hormonal levels, which in turn can affect uterine contractions and the healing process. The absence of menstruation while breastfeeding is also common due to elevated levels of prolactin, which suppresses ovulation.
However, if you notice any of the following symptoms, it is essential to seek medical attention:
1. Heavy Bleeding: If you are soaking through a pad in an hour or less for two consecutive hours, this could indicate a problem.
2. Severe Pain: While some discomfort can be expected, severe pain that does not improve with over-the-counter pain relief should be evaluated.
3. Foul Smell: A foul odor from the discharge could indicate an infection.
4. Fever: A fever over 100.4°F (38°C) could also be a sign of infection and should be addressed promptly.
In summary, while some bleeding and changes in discharge can be normal postpartum, it is crucial to listen to your body and consult with your healthcare provider if you have any concerns. Regular follow-ups with your obstetrician or midwife can help ensure that your recovery is on track and that any potential issues are addressed early. Remember, postpartum recovery is a journey, and it’s important to prioritize your health and well-being during this time.
