Is circumcision necessary?

Hello, I have a condition of phimosis where my foreskin is too long.
However, whether or not I have an erection, I can manually retract the foreskin to the coronal ridge.
When erect, the foreskin still covers the glans.
If I pull the foreskin back to the glans, there is a layer of skin that bunches at the coronal ridge, but normally it covers the glans.
I have no issues with hygiene.
Do I still need a circumcision?

Dr. Lai Ronghao reply Urology

Hello: As long as there are no issues with cleanliness and sexual intercourse is not a problem, circumcision is not necessarily required.
Thank you!

When it comes to the question of whether you need a circumcision for a long foreskin, it's important to consider several factors, including your personal comfort, hygiene, and any potential medical issues.
From your description, it seems that you have a long foreskin that can be retracted over the glans (the head of the penis) when flaccid, but it covers the glans during an erection. This condition is often referred to as "phimosis," which is when the foreskin cannot be easily retracted over the glans. However, since you mentioned that you can pull the foreskin back when not erect and that you maintain good hygiene, it may not be necessary to undergo circumcision purely for cosmetic reasons.

Here are some points to consider:
1. Hygiene: As you noted, you have no issues with cleanliness. Maintaining proper hygiene is crucial, especially if the foreskin covers the glans. Regular washing with mild soap and water can help prevent infections and other complications. If you can clean the area effectively, this is a positive sign.

2. Comfort and Functionality: If the long foreskin does not cause you any discomfort, pain, or functional issues during sexual activity, circumcision may not be necessary. Many men live comfortably with a longer foreskin without any problems.

3. Medical Considerations: If you experience any symptoms such as pain, recurrent infections, or difficulty retracting the foreskin, it would be advisable to consult a healthcare professional. They may recommend circumcision or other treatments if they believe it would alleviate your symptoms.

4. Personal Preference: Ultimately, the decision to undergo circumcision can also be influenced by personal preference or cultural/religious beliefs. If you feel that circumcision would improve your comfort or align with your values, it may be worth considering.

5. Alternatives to Circumcision: If you are hesitant about circumcision, there are non-surgical options that may help. For instance, topical steroid creams can sometimes help in cases of phimosis by thinning the foreskin, making it easier to retract. However, this should be done under the guidance of a healthcare provider.

6. Consultation with a Specialist: It is always a good idea to discuss your concerns with a urologist or a healthcare provider who specializes in male genital health. They can provide personalized advice based on your specific situation and help you weigh the pros and cons of circumcision versus other options.

In conclusion, based on your description, it does not seem that circumcision is necessary if you are comfortable and maintain good hygiene. However, consulting with a healthcare professional is the best way to ensure that you make an informed decision that is right for you. They can assess your situation more thoroughly and provide tailored recommendations.
