Regarding phimosis?

Hello, I would like to ask a question regarding my genital issue.
My foreskin can be retracted past the coronal ridge when I am not erect, but it causes a bit of pain.
However, when I am erect, I cannot retract it at all.
Even if I forcibly pull it back to the coronal ridge during an erection, it is very difficult to return it to its original position, and it is extremely painful.
Given this situation, do I need to undergo a circumcision? Additionally, would this procedure be covered by health insurance? Thank you!

Xiao Pi, 20~29 year old female. Ask Date: 2005/08/31

Dr. Xu Weikai reply Urology

Hello, based on your description, does it imply that there is a relatively narrow ring on the foreskin? If so, it is generally recommended to undergo surgery.
This is a response from Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

More Info

Phimosis is a condition characterized by the inability to retract the foreskin over the glans (the head of the penis). It can be classified into two types: physiological phimosis, which is common in infants and usually resolves naturally as they grow, and pathological phimosis, which occurs due to scarring, inflammation, or other medical conditions.
In your case, it seems you are experiencing symptoms of pathological phimosis, particularly since you mention pain when attempting to retract the foreskin, especially during an erection. This pain and difficulty in retraction can lead to complications such as infections, inflammation, or even paraphimosis, where the retracted foreskin cannot be returned to its original position, leading to swelling and potential damage to the glans.

The treatment for phimosis often depends on the severity of the symptoms and the underlying cause. In mild cases, conservative treatments such as topical corticosteroids can help to soften the foreskin and allow for easier retraction. However, in more severe cases, especially when pain is involved or if there are recurrent infections, circumcision (surgical removal of the foreskin) may be recommended.
Regarding your question about insurance coverage, in the United States, circumcision is sometimes covered by health insurance, but this can vary widely depending on the specific policy and the medical necessity of the procedure. If the phimosis is causing significant pain or complications, it is more likely to be deemed medically necessary, which could increase the likelihood of insurance coverage. It is essential to discuss your symptoms and concerns with a healthcare provider who can evaluate your condition and provide a recommendation. They can also assist you in navigating insurance coverage options.

In summary, based on your description, it would be advisable to consult with a urologist or a healthcare provider specializing in male reproductive health. They can perform a physical examination, discuss your symptoms in detail, and recommend the most appropriate treatment options. If surgery is indicated, they can also provide guidance on the insurance coverage process. Remember, addressing these concerns is important not only for physical health but also for mental well-being, as discomfort in this area can lead to anxiety and stress.
