Confusion 2

I recently noticed something unusual with my right testicle, but it doesn't seem to be the testicle itself.
I feel a sensation of pulsation in the nearby blood vessels, which feels like it's coming from within the testicle, but it's not very pronounced.
It doesn't really hurt, but it feels strange.
I had surgery for left-sided varicocele last month, and I'm wondering if this is related.
Additionally, I work in a job where I stand for over 8 hours a day.
Could you please help me understand what this might be? Thank you.

Dan Dan, 20~29 year old female. Ask Date: 2005/09/01

Dr. Xu Weikai reply Urology

Hello, after a typical varicocele surgery, it does not return to normal immediately; it may take up to 6 months for the degree of varicocele to improve.
Your condition is likely related to symptoms such as swelling and discomfort near the testicles caused by varicose veins after prolonged standing.
Sincerely, Dr.

Testicular discomfort can be concerning, and it's important to understand the potential causes and when to seek medical attention. Based on your description, it seems you are experiencing unusual sensations in your right testicle area, which you describe as a feeling of pulsation or movement, but without significant pain. You also mentioned a recent surgery for left-sided varicocele and that your job requires you to stand for long periods.

There are several possible causes for the sensations you are experiencing:
1. Varicocele: Since you recently underwent surgery for a varicocele on the left side, it is possible that you may be experiencing some residual effects or compensatory changes on the right side. A varicocele is an enlargement of the veins within the scrotum, which can lead to discomfort, a feeling of heaviness, or pulsation. If the right side is now experiencing increased blood flow or pressure due to changes from the surgery, this could explain the sensations you are feeling.

2. Epididymitis: This is an inflammation of the epididymis, which is the tube that carries sperm from the testicle. It can cause discomfort and a feeling of fullness or heaviness in the scrotum. Epididymitis can be caused by infections, including sexually transmitted infections, or by non-infectious causes such as trauma or prolonged standing.

3. Testicular torsion: Although this typically presents with severe pain and is a medical emergency, it is worth mentioning. Testicular torsion occurs when the spermatic cord becomes twisted, cutting off blood supply to the testicle. If you experience sudden, severe pain, swelling, or nausea, you should seek immediate medical attention.

4. Hernia: An inguinal hernia can cause discomfort in the groin or scrotal area, especially if there is a bulge or if the hernia becomes incarcerated or strangulated. This can occur more frequently in individuals who stand for long periods or have had previous surgeries in the area.

5. Nerve irritation: Prolonged standing can lead to nerve irritation or compression, which may cause unusual sensations in the groin or testicular area. This could be related to posture or the way pressure is distributed in your body while standing.

6. Psychological factors: Sometimes, anxiety or stress can manifest as physical sensations in the body. If you are particularly concerned about your health or have anxiety about your recent surgery, this could contribute to your perception of discomfort.

Given your symptoms and recent surgery, it is advisable to consult with a healthcare professional, preferably a urologist. They can perform a physical examination and may recommend imaging studies, such as an ultrasound, to evaluate the blood flow to the testicles and check for any abnormalities.
In the meantime, consider the following self-care measures:
- Avoid prolonged standing if possible, and take breaks to sit down and relieve pressure.

- Wear supportive underwear to help alleviate discomfort.

- Monitor your symptoms closely. If you notice any changes, such as increased pain, swelling, or changes in urination, seek medical attention promptly.

In conclusion, while there are several potential causes for the sensations you are experiencing, it is crucial to have a thorough evaluation to determine the underlying issue. Early diagnosis and treatment can help prevent complications and provide you with peace of mind.
