Testicular Discomfort: Causes and Concerns Explained - Urology

Testis

I have a part of the skin on the left side of my scrotum that itches when lightly touched, and if I scratch it, there is a sharp pain.
The skin in that area has thickened, and it feels like there is an additional layer of tissue underneath.
Additionally, when I tighten the left side of my scrotum, I can see something resembling a cluster of grapes inside (it is soft and does not hurt, but if I squeeze it too hard, I can feel pain from that specific area).
What could be the issue? Also, I often feel like I can't fully empty my bladder when urinating, and sometimes I leak urine.
Could this be related to a high frequency of masturbation? Furthermore, I notice that the time I wait to urinate is longer than average.
Please advise, thank you! ^_^

Hello POP:
1.
Based on your description of the issue with the skin on your left scrotum, it is possible that it could be chronic contact dermatitis; however, a physical examination is necessary to determine the exact problem.

2.
Your second point is somewhat difficult to understand.
Are you referring to a mass that resembles a cluster of worms? If so, this could likely indicate a varicocele.

3.
The issue with urination is not related to masturbation.
If you have psychological concerns, consider reducing the frequency of masturbation and increasing your water intake to ensure that your bladder has sufficient urine, which will help improve the force of urination.

4.
I recommend that you consult a qualified urologist for further evaluation to obtain appropriate treatment as needed.

Testicular discomfort can arise from a variety of causes, and it’s important to approach this issue with a comprehensive understanding of the potential underlying conditions. Based on your description, it sounds like you are experiencing localized itching, pain, and a thickening of the skin in the left scrotum, along with the presence of soft, grape-like structures that cause discomfort when pressed.
Firstly, the itching and pain you are experiencing could be indicative of several conditions. One possibility is a dermatological issue such as a fungal infection (like tinea cruris), which can cause itching and skin changes. Another possibility is a condition known as scrotal dermatitis, which can result from irritation, allergies, or infections. If the skin is thickening, it may also suggest a chronic inflammatory process.

The "grape-like" structures you mention could potentially be a variety of things. One common condition that could present similarly is a varicocele, which is an enlargement of the veins within the scrotum. Varicoceles are often described as feeling like a "bag of worms" and can cause discomfort, especially when pressure is applied. They are generally benign but can sometimes affect fertility.

Another possibility is the presence of an epididymal cyst or a spermatocele, which are fluid-filled sacs that can develop in the epididymis (the tube at the back of the testicle that stores and carries sperm). These cysts are usually painless but can cause discomfort if they grow large enough or if they become inflamed.

Regarding your concerns about urination, feeling like you cannot fully empty your bladder could be related to several factors, including urinary tract infections, prostate issues, or even behavioral factors such as anxiety or habit. Frequent masturbation can sometimes lead to temporary changes in urinary function, but it is unlikely to be the sole cause of your symptoms.
It is also worth noting that if you are experiencing prolonged discomfort, changes in the skin, or any new symptoms, it is crucial to consult a healthcare professional. A urologist would be best suited to evaluate your symptoms through a physical examination and possibly imaging studies or laboratory tests to determine the exact cause of your discomfort.

In summary, while there are several potential explanations for your symptoms, including dermatological issues, varicoceles, or cysts, a thorough evaluation by a healthcare provider is essential for an accurate diagnosis and appropriate treatment. Do not hesitate to seek medical attention, especially if your symptoms persist or worsen. Your health and well-being are paramount, and addressing these concerns early can lead to better outcomes.
