Keloids & Stretch Marks

Are there any methods to improve stretch marks? They seem similar to pregnancy marks.
Do dermatologists offer treatments for them? Also, I have developed keloids at the site where I received the BCG vaccine as a child.
Are there any treatment options available for that? Thank you.

nvhai, 20~29 year old female. Ask Date: 2005/09/14

Dr. Huang Ruiyun reply Dermatology

There is no definitively effective treatment for stretch marks.
The primary treatment methods for keloids include intralesional injections of medication and/or cryotherapy and/or silicone gel sheets, which can help flatten the lesions, but they cannot be completely eliminated.

More Info

Stretch marks, also known as striae, are a common skin condition that can occur due to various factors such as rapid weight gain, pregnancy, puberty, or hormonal changes. They appear as streaks or lines on the skin, often in colors ranging from red to purple, which may fade to a lighter shade over time. Keloids, on the other hand, are raised scars that form at the site of an injury or incision, and they can be more prominent in individuals with darker skin tones. Both conditions can be challenging to treat, but dermatology offers several effective treatment options.

Effective Treatments for Stretch Marks:
1. Topical Treatments:
- Retinoids: Prescription creams containing retinoids, such as tretinoin, can help improve the appearance of stretch marks by promoting collagen production and skin cell turnover. However, these should be avoided during pregnancy.

- Hyaluronic Acid: Creams or gels containing hyaluronic acid may help hydrate the skin and improve its elasticity, potentially reducing the appearance of stretch marks.

- Moisturizers: Regularly applying moisturizers can help keep the skin hydrated, which may improve its overall appearance.

2. Laser Therapy:
- Fractional Laser Resurfacing: This treatment uses laser technology to stimulate collagen production and improve skin texture. It can be effective for both new and older stretch marks.

- Pulsed Dye Laser: This type of laser targets the blood vessels in the stretch marks, helping to reduce redness and improve their appearance.

3. Microneedling:
- This procedure involves using fine needles to create micro-injuries in the skin, stimulating collagen and elastin production. It can be effective for improving the texture and appearance of stretch marks.

4. Chemical Peels:
- Chemical peels can help exfoliate the top layer of skin, promoting new skin growth and potentially improving the appearance of stretch marks.

5. Surgery:
- In severe cases, surgical options such as abdominoplasty (tummy tuck) may be considered, especially if the stretch marks are extensive and associated with excess skin.

Effective Treatments for Keloids:
1. Corticosteroid Injections:
- Injecting corticosteroids directly into the keloid can help flatten the scar and reduce inflammation. This is often the first-line treatment for keloids.

2. Surgery:
- Surgical removal of keloids can be effective, but there is a risk of recurrence. Combining surgery with other treatments, such as corticosteroid injections or radiation therapy, may help reduce the chances of the keloid returning.

3. Silicone Gel Sheets:
- Applying silicone gel sheets to the keloid can help flatten and soften the scar over time. This is a non-invasive option that can be used in conjunction with other treatments.

4. Cryotherapy:
- Freezing the keloid with liquid nitrogen can help reduce its size. This treatment is often used for smaller keloids.

5. Laser Therapy:
- Certain types of lasers can be used to reduce the redness and size of keloids, making them less noticeable.

6. Radiation Therapy:
- In some cases, low-dose radiation therapy may be used after surgical removal of a keloid to prevent recurrence.

In conclusion, both stretch marks and keloids can be effectively treated through a variety of dermatological interventions. If you are experiencing these conditions, it is essential to consult with a dermatologist who can assess your specific situation and recommend the most appropriate treatment options tailored to your needs. Early intervention often yields better results, so seeking professional advice is crucial.
