Varicose veins
Hello, doctor: I would like to ask a question.
I previously had left-sided varicocele and underwent surgery for it.
However, if my sperm motility has not increased after the surgery, or if the sperm count has not improved, what should I do? Are there other ways to remedy this? Also, how long after the surgery should I wait before having a semen analysis? During the waiting period, is it okay to have sexual intercourse or to masturbate? Thank you for your help, doctor.

Dr. Xu Weikai reply Urology
Hello, after surgery, it generally takes about 6 months for the symptoms of varicocele to resolve and heal.
Sperm motility typically improves gradually over a period of 6 to 9 months.
It is generally recommended to perform a semen analysis every 3 months post-surgery.
Whether or not to have the examination is unrelated to sexual activity; it can be done at any time you wish.

After undergoing surgery for a left-sided varicocele, it is understandable to have concerns regarding the impact on sperm quality and the recovery process. Varicocele is a condition characterized by enlarged veins within the scrotum, which can negatively affect sperm production and quality due to increased temperature and blood flow issues. Surgical intervention, typically through a procedure called varicocelectomy, aims to alleviate these problems and improve fertility outcomes.
Research indicates that approximately 60% of men experience improvements in sperm parameters, including motility and count, following varicocele surgery. However, it is important to note that not all patients will see immediate or significant improvements. Sperm quality can take time to recover post-surgery, and it may take several months for any changes to manifest in semen analysis results. Generally, it is recommended to wait about 3 to 6 months after surgery before conducting a follow-up semen analysis to accurately assess any improvements.
During the recovery period, engaging in sexual activity, including intercourse and masturbation, is typically permissible unless otherwise advised by your healthcare provider. These activities do not interfere with the healing process or sperm recovery. However, it is essential to listen to your body and avoid any activities that cause discomfort or pain in the surgical area.
If, after a reasonable recovery period, you find that there has been no improvement in sperm motility or count, it is advisable to consult with a fertility specialist or urologist. They may recommend further evaluations to identify any underlying issues that could be affecting sperm quality. Additional treatments may include lifestyle modifications, hormonal evaluations, or assisted reproductive technologies such as in vitro fertilization (IVF) with intracytoplasmic sperm injection (ICSI), which can help bypass some sperm quality issues.
In summary, while surgery for varicocele can lead to improvements in sperm quality, results can vary, and it may take time to see changes. Follow-up semen analysis is best conducted after a few months, and sexual activity is generally allowed during recovery. If concerns persist, further evaluation and alternative fertility treatments may be necessary. Always maintain open communication with your healthcare provider to ensure the best approach for your specific situation.
