I would like to inquire about bowel movement issues?

Hello, Dr.
Lin.
I would like to ask you a question on behalf of my boyfriend.
He usually has a regular bowel movement in the morning, but recently he has been experiencing a second bowel movement about half an hour after the first one.
Is this situation normal? I asked him if he feels like he hasn't fully evacuated, and whether the amount of the second bowel movement is less, but he said it varies; sometimes the second bowel movement is quite substantial.
He mentioned that when he finishes the first one, he doesn't feel like there's more to come, and he doesn't go to the bathroom due to pain—he simply feels the urge to go.
However, I feel that having two bowel movements in the morning is unusual, and I would like to understand why this might be happening.
He had surgery for internal hemorrhoids about five or six years ago and has undergone two surgeries on his stomach.
I also asked him a question regarding his hemorrhoids.
Since my mother passed away from colon cancer, I am very concerned about both my and my boyfriend's gastrointestinal health.

Hello,
1.
Each person's bowel habits and frequency vary.
An increase in bowel movements can sometimes be related to diet, emotions, lifestyle, or inflammation.
Your boyfriend has a history of gastrointestinal issues and has undergone surgery, so it is advisable for him to pay attention to his diet.
He should avoid fried and irritating foods, and instead consume more fresh and fiber-rich foods.
Establishing a daily bowel movement routine can help prevent discomfort caused by intestinal adhesions.
2.
Colorectal cancer can be associated with family history.
Common symptoms include changes in bowel habits, anemia, abdominal pain, rectal bleeding, and intestinal obstruction.
If any abnormalities occur, it is recommended to consult a gastroenterologist for evaluation.
Sincerely, Dr.

When it comes to abnormal bowel movements, understanding the underlying causes and when to seek medical advice is crucial for maintaining digestive health. Your boyfriend's situation, where he experiences two bowel movements in the morning, can be concerning, especially given your family history of colon cancer. However, it’s important to note that variations in bowel habits can occur for a variety of reasons, and not all changes indicate a serious problem.

Firstly, it is essential to consider what is considered "normal" for bowel movements. The frequency of bowel movements can vary significantly from person to person. While some individuals may have a bowel movement once a day, others may go several times a day or only a few times a week. The key factors to consider are the consistency of the stool, the presence of discomfort, and any accompanying symptoms such as pain, bloating, or changes in appetite.

In your boyfriend's case, if he is experiencing two bowel movements in the morning without any discomfort or pain, and the stool consistency remains normal, it may not be a cause for alarm. However, the fact that he has a history of gastrointestinal surgeries, including two stomach surgeries and an internal hemorrhoid surgery, could potentially influence his bowel habits. Surgical interventions can sometimes lead to changes in bowel function due to alterations in anatomy or motility.

Here are some potential explanations for his symptoms:
1. Dietary Changes: Changes in diet can significantly affect bowel habits. Increased fiber intake, for example, can lead to more frequent bowel movements. If he has recently changed his diet, this could explain the change in his bowel habits.

2. Hydration Levels: Adequate hydration is essential for healthy bowel function. If he is not drinking enough fluids, it could lead to changes in bowel habits.

3. Stress and Anxiety: Psychological factors can also play a role in bowel movements. Stress and anxiety can lead to increased bowel motility, resulting in more frequent trips to the bathroom.

4. Gastrointestinal Disorders: Conditions such as irritable bowel syndrome (IBS) or inflammatory bowel disease (IBD) can cause changes in bowel habits. If he experiences any additional symptoms, such as abdominal pain, bloating, or changes in stool consistency, it may warrant further investigation.

5. Post-Surgical Effects: Given his surgical history, it is possible that his bowel function has been altered due to changes in gut motility or structure. This could lead to variations in how often he feels the need to have a bowel movement.

Given your concerns, especially with a family history of colon cancer, it is advisable for your boyfriend to consult a healthcare professional. A thorough evaluation, including a detailed medical history and possibly diagnostic tests, can help rule out any serious conditions. The doctor may recommend a colonoscopy, especially considering his surgical history and your family history of colorectal cancer.

In summary, while two bowel movements in the morning may not be inherently abnormal, it is essential to monitor for any accompanying symptoms or changes in his overall health. Encouraging him to maintain a healthy diet, stay hydrated, and manage stress can also contribute to better digestive health. If any concerning symptoms arise, seeking medical advice promptly is the best course of action.
