Numbness in the foot?
Hello Doctor, I would like to ask about my father who is 62 years old.
When he stands, he experiences numbness on the outer side of his calf extending to his toes and thigh (the calf is the most affected), but he does not experience this numbness when sitting or walking.
It only occurs when he stands for a short period.
What could be the cause of this, and which specialist should he see? Thank you!
Xiao Ru, 30~39 year old female. Ask Date: 2005/10/03
Dr. Ye Boshou reply Neurology

More Info
Leg numbness, particularly when it occurs in specific positions like standing, can be indicative of various underlying issues. In your father's case, the numbness he experiences in the outer side of his calf, extending to his toes and thigh while standing, but not when sitting or walking, suggests a possible nerve compression or vascular issue.
Potential Causes of Leg Numbness
1. Nerve Compression: One of the most common causes of numbness in the legs is nerve compression. This can occur due to various reasons, including:
- Herniated Discs: A herniated disc in the lumbar spine can compress nerves that lead to the legs, causing numbness or tingling sensations.
- Sciatica: Compression of the sciatic nerve can lead to symptoms in the leg, particularly when standing.
- Peripheral Neuropathy: Conditions like diabetes can lead to nerve damage, resulting in numbness.
2. Circulatory Issues: Poor circulation can also lead to numbness. Conditions such as peripheral artery disease (PAD) can cause reduced blood flow to the legs, particularly when standing.
3. Musculoskeletal Issues: Conditions affecting the spine or pelvis, such as spinal stenosis or arthritis, can lead to nerve compression, resulting in numbness.
4. Postural Factors: Sometimes, simply standing in one position for too long can compress nerves or blood vessels, leading to temporary numbness.
When to Seek Help
Given that your father is experiencing numbness specifically while standing, it is advisable to seek medical attention. Here are some steps to consider:
1. Primary Care Physician: Start with a visit to a primary care physician. They can perform a thorough evaluation, including a physical examination and a review of your father's medical history.
2. Neurologist: If the primary care physician suspects a neurological issue, they may refer your father to a neurologist. A neurologist specializes in conditions affecting the nervous system and can conduct further tests, such as nerve conduction studies or electromyography (EMG), to assess nerve function.
3. Orthopedic Specialist: If there is a suspicion of musculoskeletal issues, an orthopedic specialist may be consulted. They can evaluate for conditions like herniated discs or spinal stenosis.
4. Vascular Specialist: If circulatory issues are suspected, a vascular specialist can assess blood flow to the legs and determine if there are any blockages or other vascular conditions.
Diagnostic Tests
To pinpoint the cause of the numbness, the following diagnostic tests may be recommended:
- MRI or CT Scan: Imaging studies can help visualize the spine and any potential nerve compression.
- Ultrasound: This can be used to assess blood flow in the arteries of the legs.
- Blood Tests: These can help identify underlying conditions such as diabetes or vitamin deficiencies that may contribute to neuropathy.
Conclusion
In summary, your father's leg numbness while standing could be due to nerve compression, circulatory issues, or musculoskeletal factors. It is essential to seek medical advice to determine the underlying cause and appropriate treatment. Early intervention can help manage symptoms and prevent further complications. Encourage your father to keep a record of his symptoms, including when they occur and any associated activities, as this information can be valuable for the healthcare provider in making a diagnosis.
