Urinary system issues?

Hello Dr.
Lin, I have been experiencing frequent urination for about a year, but I delayed seeing a doctor for a long time.
Initially, my condition improved, but it has been fluctuating.
During this time, I have seen a doctor several times, but my condition has not improved.
Recently, it has become increasingly severe.
I went to see a doctor and took medication for over two months, but the medication has not been very effective.
Each time I had a urinalysis, the doctor told me there was blood in my urine, and I have been experiencing frequent urination.
However, recently the situation has become even stranger, especially in the area near the urethra above the testicles (I’m not sure if that area is the urethra).
It feels like being cut with a knife, very sharp and painful, and it feels swollen.
It’s very strange; even when I go to see the doctor, I have difficulty describing that odd sensation.
It feels as if that area is somewhat numb, yet I still feel a sharp pain.
The medication prescribed by the doctor has not improved my condition at all; it seems to be completely ineffective.
Therefore, I would like to ask Dr.
Lin what my condition might be and what treatment options are available, as I am really suffering.

Hello Tim: The entire content of your question is filled with numerous typos, making it difficult to understand the core of your issue.
Besides frequent urination and the difficult-to-describe symptoms related to the urethra, do you have any other symptoms? Are you experiencing any difficulty urinating? Does your urine split into multiple streams? Do you have a strong urge to urinate? Is there an issue with urinating multiple times? You will need further examinations to determine the underlying condition, which will also provide an opportunity for further treatment.

More Info

Persistent pain and frequent urination can be indicative of several underlying urological issues, and it’s important to approach this matter with a comprehensive understanding of potential causes and treatment options. Based on your description, it seems you have been experiencing these symptoms for over a year, with varying degrees of severity. The presence of blood in your urine, along with pain near the urethra and testicular area, raises several concerns that warrant further investigation.

Possible Causes
1. Urinary Tract Infection (UTI): UTIs are common and can cause symptoms such as frequent urination, urgency, and pain during urination. The presence of blood in the urine (hematuria) can also occur. If left untreated, a UTI can lead to more severe complications.

2. Interstitial Cystitis (IC): This chronic condition is characterized by bladder pressure, bladder pain, and sometimes pelvic pain. It can lead to frequent urination and discomfort, particularly during bladder filling.

3. Urethritis: Inflammation of the urethra, often caused by infections, can lead to pain and discomfort during urination. This condition can also cause a burning sensation and may be associated with discharge.

4. Kidney Stones: These can cause severe pain, hematuria, and frequent urination. The pain often radiates from the back to the lower abdomen and can be quite severe.

5. Prostatitis: In males, inflammation of the prostate gland can lead to pain in the pelvic area, frequent urination, and discomfort during urination. Chronic prostatitis can be particularly challenging to treat.

6. Bladder or Kidney Tumors: While less common, tumors in the urinary tract can cause hematuria and pain. This is a more serious condition that requires immediate medical attention.

Recommended Actions
Given the complexity of your symptoms and the lack of improvement with medication, it is crucial to pursue further diagnostic testing. Here are some steps you should consider:
1. Consult a Urologist: If you haven’t already, seeing a urologist is essential. They specialize in urinary tract issues and can provide a more targeted approach to your symptoms.

2. Diagnostic Tests: Request a comprehensive evaluation that may include:
- Urinalysis: To check for signs of infection, blood, or other abnormalities.

- Urine Culture: To identify any bacterial infections.

- Imaging Studies: An ultrasound or CT scan can help visualize the kidneys, bladder, and urethra to check for stones or tumors.

- Cystoscopy: A procedure that allows the doctor to look inside the bladder and urethra for abnormalities.

3. Medication Review: Discuss the medications you have been prescribed. If they are not effective, your doctor may need to adjust your treatment plan. There are various classes of medications that can help manage pain and inflammation in urological conditions.

4. Lifestyle Modifications: While awaiting further evaluation, consider dietary adjustments, such as increasing fluid intake, avoiding irritants like caffeine and alcohol, and maintaining a healthy diet to support bladder health.

5. Pain Management: Over-the-counter pain relievers may provide temporary relief, but consult your doctor before starting any new medication, especially if you have ongoing symptoms.

Conclusion
Your symptoms are concerning and warrant a thorough evaluation to determine the underlying cause. Persistent pain, especially when accompanied by hematuria, should not be ignored. Early diagnosis and treatment are crucial in managing urological conditions effectively. Please ensure you communicate openly with your healthcare provider about the severity of your symptoms and the impact they have on your quality of life. This will help them tailor a treatment plan that addresses your specific needs.
