Hello, Dr.
Chuang.
I have a question I would like to ask you.
I have been experiencing itching in my vulva, and there is a lot of white discharge from my vaginal opening.
The discharge is somewhat creamy and can dampen tissue paper, but it is not tissue paper itself.
I am not sure what is going on.
A couple of days ago, I had sexual intercourse, and I am worried that I might have contracted a sexually transmitted infection (STI).
I'm very concerned.
Thank you for your help.

Baobao, 10~19 year old female. Ask Date: 2005/10/11

Dr. Zhuang Zhijian reply Obstetrics and Gynecology

Based on your description, it seems to be a vaginal candidiasis infection.
I recommend that you visit a gynecologist for diagnosis and treatment.

Vaginal itching and abnormal discharge can be concerning symptoms, and it's understandable to feel anxious about them, especially after having sexual intercourse. Let's break down the potential causes and concerns related to your symptoms.

Causes of Vaginal Itching and Discharge
1. Infections: One of the most common causes of vaginal itching and abnormal discharge is an infection. This could be a yeast infection, bacterial vaginosis, or a sexually transmitted infection (STI) such as trichomoniasis or chlamydia.
- Yeast Infections: These are caused by an overgrowth of Candida, a type of fungus. Symptoms typically include intense itching, a thick, white discharge that resembles cottage cheese, and sometimes a burning sensation during urination or intercourse.

- Bacterial Vaginosis (BV): This condition occurs when there is an imbalance in the normal bacteria found in the vagina. Symptoms may include a thin, grayish-white discharge with a fishy odor, especially after sex.

- STIs: Infections like chlamydia and gonorrhea can cause abnormal discharge and itching. These infections may also present with additional symptoms such as pelvic pain or bleeding between periods.

2. Irritation: Sometimes, irritation from soaps, detergents, or personal hygiene products can lead to itching and discomfort. This is often accompanied by a clear or slightly cloudy discharge.

3. Hormonal Changes: Fluctuations in hormones, such as those occurring during menstruation, pregnancy, or menopause, can affect vaginal discharge and may lead to itching.

4. Skin Conditions: Conditions like eczema or psoriasis can also cause itching in the genital area.

Concerns Regarding Sexual Activity
Having sexual intercourse can sometimes introduce bacteria or irritants that may lead to infections. If you have recently engaged in sexual activity and are experiencing these symptoms, it is important to consider the following:
- Risk of STIs: If you have had unprotected sex or if your partner has had multiple partners, there is a risk of STIs. It's advisable to get tested for STIs, especially if you notice any unusual symptoms.

- Communication with Partner: If you suspect an STI, it is crucial to communicate with your partner. Both of you may need to get tested and treated if necessary.

Recommendations
1. Seek Medical Attention: It is essential to consult a healthcare provider for an accurate diagnosis. They may perform a pelvic exam and take samples of the discharge for laboratory analysis to determine the cause of your symptoms.

2. Avoid Self-Medicating: While over-the-counter treatments for yeast infections are available, it’s best to avoid self-diagnosing and treating without professional guidance, as this could worsen the condition if it is not a yeast infection.

3. Maintain Hygiene: Keep the genital area clean and dry. Avoid using scented soaps or douches, as these can irritate the area further.

4. Safe Sex Practices: Use condoms to reduce the risk of STIs and practice safe sex to protect both you and your partner.

5. Monitor Symptoms: Keep track of your symptoms, including any changes in discharge, odor, or additional discomfort, and report these to your healthcare provider.

In conclusion, while vaginal itching and abnormal discharge can be caused by various factors, it is crucial to seek medical advice for proper diagnosis and treatment. Early intervention can help address any underlying issues and alleviate your concerns.
