Pain below the nipple?

Dear Doctor: I have been experiencing a slight itch at the lower edge of my nipples, and when I touch them, it turns into a sharp pain.
Sometimes it can be very painful.
This issue has been present for several years.
I would like to know what the cause might be and if it is related to breast cancer.
Thank you, and best wishes!

Understanding pain below the nipples can be a complex issue, as it may stem from various causes, ranging from benign to more serious conditions. The symptoms you describe—itching, sharp pain upon touch, and episodes of significant pain—can be concerning, especially given the duration of these symptoms over several years.
Firstly, it’s important to consider the anatomical structures in the area below the nipples. This region includes breast tissue, skin, nerves, and underlying muscles. Pain or discomfort in this area can arise from several factors:
1. Skin Conditions: Conditions such as eczema, dermatitis, or fungal infections can cause itching and discomfort. If the skin is inflamed or irritated, it may lead to pain when touched.

2. Nerve Irritation: Conditions like intercostal neuralgia, where the nerves between the ribs become irritated, can cause sharp pain that may be felt in the breast area. This could explain the pain you experience upon touch.

3. Musculoskeletal Issues: Strain or injury to the muscles in the chest wall can lead to pain that may be mistaken for breast pain. This is particularly relevant if you have engaged in activities that strain the chest muscles.

4. Hormonal Changes: Hormonal fluctuations, especially those related to the menstrual cycle, can lead to breast tenderness and pain. This is often cyclical and can be accompanied by other symptoms such as swelling or sensitivity.

5. Breast Conditions: Benign breast conditions such as fibrocystic changes or breast cysts can also cause pain and discomfort. These conditions are generally not serious but can lead to significant discomfort.

6. Infections: Infections such as mastitis, which is more common in breastfeeding women, can cause localized pain and tenderness in the breast area.

7. Cancer Concerns: While breast cancer can present with pain, it is typically associated with other symptoms such as a lump, changes in breast shape or size, or changes in the skin over the breast. It’s important to note that breast cancer is more commonly associated with painless lumps, but any persistent pain, especially if it has changed over time, should be evaluated.

Given that your symptoms have persisted for several years, it is crucial to seek a thorough evaluation from a healthcare professional. A clinical examination, possibly including imaging studies like a mammogram or ultrasound, may be warranted to rule out any serious conditions, including breast cancer.
In summary, while your symptoms could be related to benign conditions, the duration and nature of the pain warrant further investigation to ensure that there are no underlying serious issues. It is always better to err on the side of caution when it comes to breast health. Please consult with a healthcare provider who can perform a detailed assessment and provide you with the appropriate care and guidance.
