Gastrointestinal discomfort?
Hello, I have been experiencing chronic constipation, and drinking yogurt has not been effective.
Sometimes using glycerin does not work either, which is quite severe.
I had an endoscopy that showed gastric inflammation, and as a result, I have a strong odor in my saliva, which is noticeable even when I breathe.
People sitting near me can smell it, and this is very distressing for me.
I would like to know how to resolve this issue.
I have consulted with doctors at Cathay and Chang Gung hospitals and took medication for two months, but there has been no improvement.
Thank you for your response.

Dr. Lin Minghui reply Gastroenterology and Hepatology
Hello, long-term constipation, poor appetite, and gastrointestinal discomfort should prompt an evaluation to rule out any gastrointestinal lesions or tumors.
Additionally, constipation can be related to emotional factors, medications, and endocrine disorders.
It is advisable to continue seeking medical attention.
In the meantime, it is recommended to drink plenty of water, consume more fiber (such as vegetables and fruits), limit spicy foods, maintain a positive mood, establish a daily bowel habit, and engage in moderate exercise.
Sincerely, Dr.

More Info
Chronic constipation and gastrointestinal discomfort can be challenging conditions to manage, especially when they persist despite various treatments. Based on your description, it seems you are experiencing significant discomfort, including severe constipation and issues related to gastritis, which may be contributing to bad breath and other symptoms. Here are some effective strategies and solutions that may help alleviate your symptoms.
Understanding Your Symptoms
Chronic constipation can be caused by a variety of factors, including dietary habits, lack of physical activity, dehydration, and underlying medical conditions. In your case, the presence of gastritis (inflammation of the stomach lining) may also be affecting your digestive health. The heavy taste in your mouth and noticeable bad breath could be linked to poor digestion or the buildup of bacteria in the stomach due to delayed gastric emptying.
Dietary Modifications
1. Increase Fiber Intake: Aim for a diet rich in fiber, which can help promote regular bowel movements. Foods high in fiber include fruits (like apples, pears, and berries), vegetables (such as broccoli, carrots, and leafy greens), whole grains (like oats, brown rice, and whole wheat bread), and legumes (beans and lentils). Gradually increase your fiber intake to avoid gas and bloating.
2. Stay Hydrated: Drinking plenty of water is crucial for preventing constipation. Aim for at least 8-10 cups of water daily. Herbal teas and clear broths can also contribute to your fluid intake.
3. Probiotics: Since you mentioned consuming yogurt, ensure that it contains live active cultures, which can help balance gut bacteria and improve digestion. You might also consider probiotic supplements, which can be beneficial for gut health.
4. Avoid Trigger Foods: Certain foods can exacerbate gastrointestinal discomfort, such as spicy foods, fatty foods, and processed foods. Keep a food diary to identify any specific triggers that worsen your symptoms.
Lifestyle Changes
1. Regular Exercise: Physical activity can stimulate bowel function and help alleviate constipation. Aim for at least 30 minutes of moderate exercise most days of the week. Activities like walking, jogging, or yoga can be particularly beneficial.
2. Establish a Routine: Try to set a regular time each day for bowel movements. This can help train your body to have more consistent bowel habits.
3. Stress Management: Stress can significantly impact gastrointestinal health. Consider incorporating relaxation techniques such as deep breathing, meditation, or yoga into your daily routine.
Medical Management
1. Medications: If dietary changes and lifestyle modifications do not provide relief, it may be necessary to consult with a gastroenterologist for further evaluation. They may recommend medications such as osmotic laxatives (e.g., polyethylene glycol) or stimulant laxatives (e.g., bisacodyl) for more severe cases of constipation.
2. Review Current Medications: If you are taking medications for gastritis or other conditions, discuss with your doctor whether any of these could be contributing to your constipation or gastrointestinal discomfort.
3. Follow-Up Care: Since you have already seen specialists at Cathay and Chang Gung hospitals, it may be beneficial to schedule a follow-up appointment to discuss your ongoing symptoms and explore additional diagnostic tests if necessary.
Conclusion
Chronic constipation and gastrointestinal discomfort can be complex issues that require a multifaceted approach to treatment. By making dietary adjustments, increasing physical activity, managing stress, and working closely with your healthcare providers, you can develop a personalized plan to improve your symptoms. If your condition does not improve, do not hesitate to seek further medical advice, as there may be underlying issues that need to be addressed. Remember, it’s essential to be patient and persistent in finding the right combination of strategies that work for you.
