Post-Surgery Pain: Complications After Ear Surgery - Surgery

Post-surgery

Doctor, I previously had surgery for a preauricular sinus at Changhua Hospital.
The physician said I was healed.
After returning home, I initially thought I was fine, but recently the area where I had the surgery has suddenly become very painful, and I also have a headache (not sure if it's related).
I have tried to squeeze it, and there was an unpleasant odor, but I didn't see anything come out.
Could it be that I have relapsed? Please help.

Dr. Lai Jihong reply Surgery

Hello,
The causes of preauricular fistula can be either congenital or acquired.
Generally, if the lesion has been present for a long time, the acquired lesions may initially arise from infections of sebaceous cysts or traumatic infections, which, if not completely healed, can lead to the formation of infectious sinuses or open fistulas in the subcutaneous or deeper tissues.
The recurrence rate after fistula surgery is approximately 10-20%.
This is related to the long-term infection infiltrating the surrounding tissues, leading to recurrent tissue infections.
It is recommended to return to the surgical outpatient clinic for follow-up and further explanation and treatment.
Wishing you good health,
Lai Ji-Hong

Post-surgery pain, especially after ear surgery, can be concerning and may indicate complications that need to be addressed. Based on your description, it seems you are experiencing significant pain at the surgical site, along with headaches and an unusual odor, which could suggest a few potential issues.

Firstly, it is important to understand that pain following surgery can be a normal part of the healing process. However, if the pain is severe and persistent, it may indicate complications such as infection or the formation of a seroma (a pocket of fluid that can develop after surgery). The presence of an unusual odor is particularly concerning, as it may suggest an infection. Infections can occur if bacteria enter the surgical site, leading to symptoms such as increased pain, swelling, redness, and discharge.

Given that you mentioned squeezing the area and noticing an odor without seeing any discharge, it is possible that there is a buildup of pus or fluid that is not readily visible. This can happen in cases where the surgical site has not healed properly, or if there is a blockage preventing drainage. The fact that you are experiencing headaches could also be related to the pain from the surgical site, as pain can often radiate and cause discomfort in other areas, including the head.

In your situation, it is crucial to follow up with your surgeon or healthcare provider as soon as possible. They will be able to conduct a thorough examination of the surgical site to determine if there is an infection or another complication that requires treatment. This may include imaging studies, such as an ultrasound or CT scan, to assess the area for any fluid collections or other abnormalities.

In the meantime, it is advisable to avoid squeezing or manipulating the area further, as this can exacerbate any potential infection or irritation. Keeping the area clean and dry is essential, and you should follow any post-operative care instructions provided by your surgeon.

Pain management is also an important aspect of recovery. If you are experiencing significant discomfort, discuss with your healthcare provider about appropriate pain relief options. Over-the-counter medications like ibuprofen or acetaminophen can help, but your provider may also prescribe stronger pain relief if necessary.

In summary, while some pain after ear surgery can be expected, the severity of your symptoms, particularly the odor and headaches, warrants immediate medical attention. Prompt evaluation by your healthcare provider will help ensure that any complications are addressed quickly, allowing for a smoother recovery process. Remember, it is always better to err on the side of caution when it comes to post-surgical symptoms.
