Severe Acne Woes: Seeking Effective Treatment and Solutions - Dermatology

Acne

Hello Doctor: I would like to ask about my acne situation.
I used to have acne, but it was not very severe.
However, I don't know why it has become very serious in the past two to three months, with many breakouts.
Many people have asked me why my acne is so severe.
I went to see a dermatologist, but it did not improve; in fact, it got worse.
Before the visit, I only had acne, but after the appointment, not only did my face not improve, but I also developed even more acne, and my face became red and swollen.
The acne sometimes bursts and bleeds, and I am afraid of developing cellulitis, so I have been wearing a mask.
Will this have any impact? I am also hesitant to return to the same dermatologist, and switching to another one yielded similar results, as the doctors did not provide much guidance, only advising me to sleep earlier and avoid oily and fried foods.
I have been following this advice, but it has only improved slightly.
Now, I have not only acne but also wounds and scabs on my face, which are painful when I wash my face.
I am reluctant to wash my face at all.
What should I do? I really feel helpless and am starting to wonder if there is something wrong with my body and if I should get a health check-up.
P.S.
If I go to Taipei, where would be a good place to seek help?

Moderate to severe acne requires treatment for at least three months before noticeable results can be seen, so attending just one or two outpatient visits is not very effective.
If you are in the Taipei area, you can follow up with treatment at a public hospital or a non-profit hospital's dermatology department based on your convenience.
As for the various factors that may worsen acne, it is not possible to analyze them online; it is best to consult with the physician who is treating you.

More Info

Severe acne can be a distressing condition, especially when it escalates to the point of causing physical pain and emotional distress. Your experience of worsening acne, along with the development of open sores and scabbing, suggests that you may be dealing with a more severe form of acne, such as cystic acne or possibly even secondary infections. It’s understandable that you feel overwhelmed and uncertain about the next steps to take.

First and foremost, it’s crucial to understand that acne can be influenced by a variety of factors, including hormonal changes, diet, stress, and even certain medications. The fact that your acne has worsened significantly over the past few months may indicate a need for a more comprehensive evaluation. It’s also concerning that your skin has become red, swollen, and painful, which could potentially lead to complications such as cellulitis (a skin infection) if not addressed properly.

Given your situation, here are several recommendations:
1. Consult a Dermatologist: It’s essential to find a dermatologist who specializes in acne treatment. If your previous consultations did not yield satisfactory results, consider seeking a second opinion from a different dermatologist. Look for one who has experience with severe acne cases and is willing to explore various treatment options, including prescription medications.

2. Treatment Options: There are several effective treatments for severe acne, including:
- Topical Treatments: Retinoids, benzoyl peroxide, and antibiotics can help reduce inflammation and bacteria on the skin.

- Oral Medications: Depending on the severity, your dermatologist may prescribe oral antibiotics, hormonal treatments (like birth control pills for women), or isotretinoin (commonly known as Accutane), which is effective for severe cases.

- Chemical Peels or Laser Therapy: These treatments can help reduce acne scarring and improve skin texture.

3. Skin Care Routine: While it may be painful, maintaining a gentle skincare routine is important. Use a mild cleanser and avoid scrubbing your face, which can exacerbate irritation. Look for non-comedogenic products that won’t clog pores.

4. Diet and Lifestyle: While dietary changes can help some individuals, they may not be the sole solution. However, maintaining a balanced diet, staying hydrated, and managing stress through activities like yoga or meditation can contribute positively to your skin health.

5. Avoid Picking or Squeezing: It’s crucial to avoid picking at your acne or scabs, as this can lead to scarring and increase the risk of infection.

6. Health Check-Up: If you suspect that there may be underlying health issues contributing to your skin problems, it may be worthwhile to undergo a general health check-up. Hormonal imbalances, for example, can significantly affect skin health.

7. Infection Concerns: If you are worried about developing cellulitis or any other infections, do not hesitate to seek immediate medical attention. Signs of infection include increased redness, warmth, swelling, and pus formation.

8. Mask Usage: Wearing a mask can help protect your skin from environmental irritants, but ensure that the mask is clean and breathable to avoid trapping moisture and bacteria against your skin.

In terms of where to seek treatment in Taipei, consider visiting reputable dermatology clinics or hospitals known for their dermatology departments. Research online reviews and ask for recommendations from friends or family who may have had positive experiences.

In conclusion, while severe acne can be frustrating and disheartening, there are effective treatments available. It’s important to be proactive in seeking the right care and to communicate openly with your healthcare providers about your concerns and treatment goals. Remember, you are not alone in this, and with the right support and treatment plan, you can achieve clearer skin.
