Concerns About Testicular Size and Breast Changes: What You Need to Know - Urology

Question

Hello Doctor: I have noticed that my testicles have not significantly increased in size over the past two to three years, measuring about 3.2 cm in length, with the left one being slightly smaller.
Additionally, there is a hard lump about 2 cm in size beneath the nipple of my right breast, which sometimes hurts when pressed and sometimes does not.
The entire right breast also appears larger, resembling female development.
I am very concerned about this...
Will it affect my fertility? Furthermore, I have noticed some white substances in my semen; what could they be? Are they produced by the testicles? Thank you for taking the time to respond!

Xiao Xiao, 10~19 year old female. Ask Date: 2005/10/29

Dr. Xu Weikai reply Urology

Hello, regarding the size of the testicles, unless they are very small and lack firmness (meaning they are too soft), their general function is considered normal.
If you are concerned, you can undergo an examination, including a testicular ultrasound, male hormone levels, and semen analysis, which can provide the most accurate diagnosis.
As for breast issues, sometimes they are related to puberty and can temporarily cause breast pain and the presence of lumps.
If you are worried, you can seek medical attention nearby.

Sincerely,
Dr.

It sounds like you have several concerns regarding changes in your breast and testicular size, as well as some unusual findings in your semen. Let's break down your concerns and address them one by one.

Firstly, regarding the changes in your breast, particularly the right side which has a noticeable hard lump and is larger than the left side, it is important to take these symptoms seriously. The presence of a hard lump that causes pain intermittently could indicate a variety of conditions, ranging from benign issues such as gynecomastia (enlargement of breast tissue in males) to more serious concerns like a potential tumor. Gynecomastia can occur due to hormonal imbalances, often related to testosterone and estrogen levels. It is not uncommon for males to experience breast tissue changes, especially during puberty or due to certain medications, health conditions, or lifestyle factors.

Given the size difference and the presence of a hard lump, it is highly advisable to consult a healthcare professional, preferably a specialist in breast health or an endocrinologist. They may recommend imaging studies, such as an ultrasound or mammogram, to further evaluate the lump and assess the breast tissue. Early evaluation is crucial, especially since you mentioned that the lump can be painful at times.

Now, regarding your testicular size, a measurement of about 3.2 cm is within the normal range for adult males, but size can vary significantly among individuals. If you have concerns about testicular size or if you notice any changes in texture, shape, or if you experience pain, it is important to seek medical advice. Testicular health is vital for fertility, and any abnormalities should be evaluated by a healthcare provider.

As for the white substances in your semen, this could be due to several factors. Semen typically contains sperm, seminal fluid, and various other components that can appear white or cloudy. If you notice a significant change in the appearance of your semen, such as a persistent change in color, consistency, or if you experience pain during ejaculation, it would be wise to consult a urologist. They can perform tests to rule out infections or other conditions that could affect your reproductive health.

In summary, your concerns about breast changes, testicular size, and semen appearance warrant a thorough evaluation by a healthcare professional. It is understandable to feel anxious about these changes, but seeking medical advice is the best course of action. A healthcare provider can offer you a proper diagnosis and treatment options if necessary. Remember, it is always better to address health concerns early rather than waiting for symptoms to worsen. Don't hesitate to reach out to a specialist who can help you navigate these issues with the care and attention they deserve.
