The discharge before menstruation can have an unpleasant odor and may be watery?
Hello, I have had an intrauterine device (IUD) for over a year.
Recently, in the days leading up to my period, I have noticed an increase in vaginal discharge that is more watery than usual, but it has a very strong and unpleasant odor.
I also experience abdominal pain, but there is not much discoloration; it usually takes until the third day for my period to start with a red flow.
This has never happened before.
I went to the hospital for an ultrasound, and the IUD is in place.
However, in the days before my period, the discharge is foul-smelling, and it stops after my period ends.
What could be the issue, and what can I do to improve this situation? Thank you for your help.

Hello: If there is a fishy-smelling discharge from the vagina, the most likely diagnosis is bacterial vaginosis.
Please seek medical attention promptly.

It sounds like you are experiencing some concerning symptoms related to unusual vaginal discharge, particularly before your menstrual period. Given that you have an intrauterine device (IUD) in place, it's important to consider several factors that could be contributing to your symptoms.
Firstly, the increase in watery discharge that has a strong odor could indicate a possible infection, such as bacterial vaginosis (BV) or a sexually transmitted infection (STI). Bacterial vaginosis is characterized by an imbalance in the normal bacteria found in the vagina, leading to an increase in discharge that can have a fishy odor, particularly after sexual intercourse. STIs, such as trichomoniasis, can also cause similar symptoms, including unusual discharge and odor.
The fact that you experience this discharge primarily before your menstrual period may also be related to hormonal changes. The hormonal fluctuations that occur in the menstrual cycle can affect the vaginal flora and lead to changes in discharge. However, the presence of a strong odor is not typical and should be evaluated further.
Additionally, the pain you are experiencing in your abdomen could be related to the same underlying issue. Infections can cause pelvic pain, and if the infection spreads, it may lead to more severe symptoms. It's crucial to monitor the pain and any other symptoms you may experience.
Since you mentioned that you have already had an ultrasound and that your IUD is in place, it would be wise to follow up with your healthcare provider for a thorough examination. They may recommend a pelvic exam and tests to check for infections. A swab of the vaginal discharge can help identify any bacterial or fungal infections that may be present.
In terms of management, if an infection is diagnosed, your healthcare provider will likely prescribe antibiotics or antifungal medications, depending on the cause. It's essential to complete the full course of any prescribed medication, even if symptoms improve before finishing the treatment.
To improve your symptoms and overall vaginal health, consider the following tips:
1. Maintain Good Hygiene: Keep the vaginal area clean and dry. Avoid using scented soaps or douches, as these can disrupt the natural balance of bacteria.
2. Wear Breathable Underwear: Cotton underwear allows for better airflow and can help reduce moisture buildup.
3. Avoid Irritants: Stay away from products that may irritate the vaginal area, such as scented pads or tampons.
4. Stay Hydrated: Drinking plenty of water can help flush out toxins and support overall health.
5. Regular Check-ups: Continue to have regular gynecological exams, especially if you notice any changes in your discharge or experience new symptoms.
If your symptoms persist or worsen, do not hesitate to seek medical attention. It's essential to address any potential infections or underlying issues promptly to avoid complications. Your health and comfort are paramount, so please take these symptoms seriously and consult with your healthcare provider for a comprehensive evaluation and appropriate treatment.
