Unidentified mass in the right side of the neck?
Hello, Dr.
Hsu: I have been experiencing nasal congestion for some time.
Recently, I noticed blood in my nasal mucus, which is a small amount mixed in.
Additionally, I have an unexplained lump on the right side of my neck, located about seven to eight centimeters behind my right ear.
It does not hurt when I press on it, but I feel it is getting larger; about a year ago, it was just a small hard lump that I didn't think much of, but now it feels concerning.
Could this be indicative of any disease? Is it possible that it could be nasopharyngeal cancer? Thank you for your response, Dr.
Hsu!

Hello: In response to your question, nasal congestion and blood-tinged nasal discharge can indeed be symptoms of nasopharyngeal carcinoma, but they may also be caused by other issues.
Additionally, when there is an unexplained lump under the ear along with these symptoms, we should certainly rule out the worst-case scenario first.
Since you suspect nasopharyngeal carcinoma, it is advisable to consult a physician.
A simple examination and biopsy can provide a quick diagnosis.
The cure rate for early-stage nasopharyngeal carcinoma is very high, so don't miss the opportunity!

Dear Patient,
Thank you for your inquiry regarding the combination of unexplained neck lumps and bloody nasal discharge. Your symptoms raise several important considerations that warrant a thorough evaluation.
Firstly, the presence of bloody nasal discharge can be attributed to various causes. Commonly, nasal bleeding occurs due to trauma, dry air, or underlying conditions such as hypertension or nasal infections. Given that you have a history of nasal congestion, it is possible that the blood is a result of irritation or damage to the nasal mucosa, especially if you have been experiencing frequent nasal obstruction and possibly forceful nose blowing. The blood you describe as being "adhered to the mucus" suggests that it may not be a significant hemorrhage but rather localized bleeding from a small area in the nasal cavity.
On the other hand, the neck lump you mentioned, located near the right ear and progressively increasing in size, is particularly concerning. Lumps in the neck can arise from various etiologies, including benign conditions such as lymphadenopathy (swollen lymph nodes), cysts, or lipomas. However, given the duration of your symptoms and the fact that the lump has grown over time, it is crucial to consider more serious conditions, including malignancies such as nasopharyngeal carcinoma (NPC).
Nasopharyngeal carcinoma is a type of cancer that originates in the nasopharynx, the area behind the nose and above the back of the throat. It is more common in certain populations and can present with symptoms such as nasal obstruction, bloody nasal discharge, and neck lumps due to lymph node involvement. The presence of a neck lump, particularly one that is firm and increasing in size, raises suspicion for possible metastatic disease from a primary tumor in the nasopharynx.
Given your concerns and the combination of symptoms, I strongly recommend that you seek further evaluation from an Ear, Nose, and Throat (ENT) specialist. A thorough examination, including nasal endoscopy, imaging studies (such as a CT scan of the neck and nasopharynx), and possibly a biopsy of the neck lump, will be essential in determining the underlying cause of your symptoms. Early diagnosis is crucial, especially if there is a possibility of malignancy.
In summary, while the bloody nasal discharge may be related to benign causes, the progressive neck lump necessitates further investigation to rule out serious conditions, including nasopharyngeal carcinoma. Please do not delay in seeking medical attention, as timely intervention can significantly impact outcomes.
Wishing you good health and a prompt resolution to your symptoms.
Best regards,
