Abdominal pain
Hello Doctor: After having sexual intercourse the night before last, I experienced abdominal pain (the pain felt like needing to use the restroom).
I thought I needed to go to the bathroom, so I spent the whole night straining to have a bowel movement, but still felt like I couldn't empty my bowels completely.
During the night, the abdominal pain was severe and intermittent.
The next day, I went to see a doctor, and after an abdominal ultrasound, it was found that there was bleeding.
At that time, the doctor asked if I had vaginal bleeding, but I did not! The doctor said that the bleeding was caused by vigorous sexual activity and prescribed me ColioPAN and Ponstan.
Today, I noticed that my discharge has blood in it.
Can these two medications treat the bleeding in my discharge? Should I seek medical attention immediately, or should I just observe? My abdomen still hurts near the navel, sometimes with intermittent pain, and pressing on my abdomen is painful.
I often have gastrointestinal issues, and I'm not sure if it's intestinal pain or a gynecological problem...
What should I do???

Hello: Sexual activity can cause rupture of the ovarian corpus luteum, which may lead to intra-abdominal bleeding.
If the bleeding is minimal, the pain in the abdomen will gradually improve after a few days as the blood is absorbed.
However, if the bleeding continues, it may result in dizziness and severe abdominal pain, which could necessitate surgery to stop the bleeding.
If you experience dizziness or worsening abdominal pain, you should immediately return to the emergency department; otherwise, follow the instructions of your outpatient physician.

More Info
Experiencing abdominal pain after sexual activity can be concerning and may arise from various causes. Based on your description, it seems that you have had intense sexual activity, which has led to abdominal pain and some bleeding. Here’s a comprehensive understanding of what might be happening and what steps you should consider taking.
Possible Causes of Abdominal Pain After Sexual Activity
1. Pelvic Congestion: Engaging in intense sexual activity can lead to increased blood flow to the pelvic region, which may cause a feeling of fullness or pressure. This can sometimes result in pain, especially if there is underlying pelvic congestion.
2. Vaginal or Cervical Trauma: Even without penetration, vigorous sexual activity can cause trauma to the vaginal walls or cervix, leading to bleeding and pain. If you experienced any discomfort during the activity, it could indicate that some trauma occurred.
3. Endometriosis or Ovarian Cysts: Conditions like endometriosis or the presence of ovarian cysts can cause significant pain during or after sexual activity. These conditions can lead to inflammation and pain in the pelvic region.
4. Infection: Pelvic inflammatory disease (PID) or urinary tract infections (UTIs) can also cause abdominal pain and may be exacerbated by sexual activity. Symptoms may include pain, bleeding, and unusual discharge.
5. Gastrointestinal Issues: Sometimes, the pain might not be directly related to the reproductive organs but rather to gastrointestinal issues. The sensation of needing to have a bowel movement could indicate gastrointestinal distress, which can be confused with pelvic pain.
Medications and Their Role
You mentioned being prescribed ColioPAN and Ponstan. ColioPAN is typically used for gastrointestinal discomfort, while Ponstan is a non-steroidal anti-inflammatory drug (NSAID) that can help alleviate pain and reduce inflammation. While these medications can help manage pain, they may not address the underlying cause of your symptoms, especially if there is an infection or significant trauma.
Recommendations
1. Monitor Symptoms: Keep track of your symptoms, including the intensity and frequency of the pain, any changes in bleeding, and any new symptoms that may arise.
2. Seek Medical Attention: Given that you are experiencing ongoing abdominal pain, bleeding in your discharge, and a history of gastrointestinal issues, it is crucial to seek medical attention. A healthcare provider can perform a thorough examination, possibly including pelvic exams, ultrasounds, or other imaging studies, to determine the cause of your symptoms.
3. Avoid Sexual Activity: Until you have a clearer understanding of what is causing your pain and bleeding, it may be wise to refrain from sexual activity to prevent further irritation or injury.
4. Consider a Specialist: If your symptoms persist or worsen, consider consulting a gynecologist or a gastroenterologist, depending on whether the pain seems more related to reproductive or gastrointestinal issues.
5. Hydration and Rest: Ensure you are well-hydrated and get plenty of rest as your body recovers. This can help alleviate some discomfort.
Conclusion
Abdominal pain following sexual activity can stem from various causes, and it is essential to approach this issue with care. While medications may provide temporary relief, they do not replace the need for a thorough medical evaluation. Given your symptoms, it is advisable to seek medical attention promptly to rule out any serious conditions and to receive appropriate treatment. Your health and well-being are paramount, and addressing these symptoms early can help prevent further complications.
