Varicose veins?

Hello doctor, I have a question.
My ankles and toes have become dark due to varicose veins, and my toenails seem to have changed color as well.
However, the veins in my calves are not bulging out like a severe case.
Do I need surgery for this?

Hello: Since this is related to cardiovascular surgery, a cardiovascular surgeon will respond to you! Based on your description, it may be varicose veins accompanied by hyperpigmentation and stasis dermatitis.
Please come to the outpatient clinic for a physical examination and necessary tests, after which we can determine if surgical treatment is needed.
The cardiovascular surgery outpatient clinic is open every Wednesday morning and afternoon.
Wishing you good health!

More Info

When considering whether surgery is necessary for varicose veins, especially in the context of discolored toes and ankles, it is essential to evaluate several factors. Varicose veins are enlarged, twisted veins that often appear blue or dark purple, and they can lead to various complications, including skin changes, ulcers, and blood clots. The discoloration of your toes and ankles could indicate poor circulation or venous insufficiency, which may require medical attention.

Understanding Varicose Veins and Symptoms
Varicose veins occur when the valves in the veins do not function properly, causing blood to pool in the veins instead of flowing back to the heart. This can lead to symptoms such as:
- Swelling in the legs or ankles
- Pain or heaviness in the legs
- Discoloration of the skin, particularly around the ankles
- Itching or irritation over the affected veins
- Skin ulcers or sores in severe cases
The discoloration of your toes and ankles, particularly if they appear dark or have changed color, can be a sign of chronic venous insufficiency. This condition can lead to skin changes and, if left untreated, may progress to more severe complications.

When is Surgery Necessary?
Surgery for varicose veins is typically considered when:
1. Symptoms are Severe: If you experience significant pain, swelling, or discomfort that interferes with daily activities, surgical intervention may be warranted.

2. Skin Changes: The presence of discoloration, ulcers, or other skin changes can indicate that the condition is worsening and may require surgical treatment to prevent further complications.

3. Failure of Conservative Treatments: If you have tried conservative treatments such as compression stockings, lifestyle changes, and medications without relief, surgery may be the next step.

4. Risk of Complications: If there is a risk of developing blood clots or other serious complications due to the varicose veins, surgical options may be recommended.

Types of Surgical Treatments
If surgery is deemed necessary, there are several options available:
- Endovenous Laser Treatment (EVLT): A minimally invasive procedure that uses laser energy to close off varicose veins.

- Sclerotherapy: Involves injecting a solution into the vein, causing it to collapse and fade from view.

- Vein Stripping: A more traditional surgical approach where the affected vein is removed through small incisions.

- Ambulatory Phlebectomy: Involves removing smaller varicose veins through tiny punctures in the skin.

Consultation with a Specialist
Given your symptoms, it is crucial to consult with a vascular specialist or a healthcare provider who can perform a thorough evaluation, including a physical examination and possibly an ultrasound to assess the blood flow in your veins. They can help determine the severity of your condition and whether surgery is the best option for you.

Conclusion
In summary, while not all cases of varicose veins require surgery, the presence of discolored toes and ankles suggests that your condition may be more serious and warrants further investigation. It is essential to seek medical advice to evaluate your symptoms and discuss potential treatment options. Early intervention can help prevent complications and improve your quality of life.
