Ankle fracture
I have had an ankle fracture for over two months, but I still feel pain in the ankle joint area when walking.
What could be the reason? Both the inner and outer ankle bones are fractured, but I can walk now.
However, the joint hurts only when I walk; it doesn't hurt when I'm not walking.
Is there a problem? Could it be nerve damage?

Dr. Shi Guozheng reply Orthopedics
After two months of an ankle fracture, there should be callus formation; however, complete healing may still take another month or two.
Therefore, experiencing pain while walking is a normal phenomenon.
If both the medial and lateral malleoli are fractured and it is not an open fracture, it typically does not damage the nerves, so the pain is unrelated to nerve injury.

More Info
After experiencing an ankle fracture, it is not uncommon to encounter pain during the recovery process, even two months post-injury. The pain you are feeling in the ankle joint while walking could be attributed to several factors, and understanding these can help alleviate your concerns.
Firstly, it is important to recognize that pain during the recovery phase is often a normal part of the healing process. When bones fracture, the surrounding tissues, including ligaments, tendons, and muscles, can also be affected. This can lead to inflammation and discomfort as the body works to heal itself. In your case, since both the inner and outer ankle bones were fractured, the surrounding soft tissues may still be healing, which can cause pain during weight-bearing activities like walking.
Secondly, the pain you experience while walking but not at rest suggests that it may be related to the mechanical stress placed on the ankle joint during movement. When you walk, the forces acting on the ankle joint increase, which can exacerbate any lingering inflammation or irritation in the area. This is particularly true if you have recently begun to increase your activity level or if you are engaging in rehabilitation exercises that put stress on the ankle.
Another possibility is that the pain could be due to stiffness in the joint. After a period of immobilization, such as wearing a cast or splint, it is common for joints to become stiff. This stiffness can lead to discomfort when the joint is used, especially if you are not yet fully accustomed to bearing weight on it again. Engaging in physical therapy or rehabilitation exercises can help improve flexibility and strength, which may alleviate some of the pain.
Regarding your concern about nerve damage, while it is possible for a fracture to impact nearby nerves, this is less common. If the pain is sharp, burning, or accompanied by numbness or tingling, it may warrant further investigation to rule out nerve involvement. However, if the pain is more of a dull ache or discomfort localized to the joint, it is likely related to the healing process rather than nerve damage.
In terms of recovery, it is essential to follow up with your healthcare provider or physical therapist. They can assess your ankle's range of motion, strength, and overall function. If the pain persists or worsens, they may recommend imaging studies, such as an X-ray or MRI, to ensure that there are no complications, such as nonunion of the fracture or other injuries that may have gone unnoticed.
In summary, experiencing pain in your ankle two months after a fracture can be a normal part of the healing process, particularly if you have recently resumed walking. Factors such as inflammation, joint stiffness, and the mechanical stress of walking can all contribute to your discomfort. It is advisable to continue with rehabilitation exercises, monitor your symptoms, and consult with your healthcare provider for personalized guidance and to ensure a safe and effective recovery.
