Urethral Sensitivity and Pain During Urination - Urology

Urinary issues?

I have a question.
When I urinate, I feel a lot of sensitivity and pain in the urethra at the front part of the penis (near the glans), which makes me hesitant to urinate.
This has caused me to only be able to urinate in small, intermittent amounts or to rely on contractions of the perineum to expel urine.
This has been going on for several years.
I have visited many hospitals and undergone numerous tests, all of which have come back normal, including a cystoscopy.
However, I remember that around the time I was in sixth grade, I forcefully retracted my foreskin during masturbation and exposed the entire glans (although it was painful at the time).
The next morning, I noticed that the foreskin did not naturally return to its original position (the entire glans was still visible), and the urethral opening appeared to be noticeably elongated.
The elongated urethral opening was red and painful to the touch.
Every time I had an erection and retracted the foreskin, the frenulum felt very uncomfortable.
After this incident, I noticed that at the end of urination, the urine felt like it was getting stuck and I had to manually push and pull the foreskin to expel the last drops of urine (which I have to do every time).
Despite this, I still have the habit of masturbation.
So, I would like to know if this could be considered a frenulum injury and if it could be the reason for my long-term urinary difficulties.
I kindly ask for your help, doctor.

Dr. Xu Weikai reply Urology

Xiao Zhang: Hello, I believe it is a localized chronic inflammation, commonly referred to as fibrosis.
Generally, no special treatment is required; symptomatic treatment is sufficient.
If there is a real issue, I recommend undergoing urodynamic testing, which is currently available at major hospitals.
This can help determine if there are any associated neurological or muscular connection problems.
This is a response from Dr.
Hsu Wei-Kai, Director of the Urology Department at Taitung Hospital, Ministry of Health and Welfare.

Understanding urethral sensitivity and pain during urination can be quite complex, especially when there are historical factors involved, such as the incident you described during your childhood. Based on your symptoms and history, it seems you may be experiencing a combination of physical and possibly psychological factors contributing to your discomfort.

Firstly, the sensitivity and pain you feel at the tip of the penis during urination could be due to a few reasons. One possibility is that there is some form of irritation or inflammation in the urethra or surrounding tissues. This could be a result of a previous injury, such as the one you described when you forcefully retracted the foreskin. This kind of trauma can lead to scarring or changes in sensitivity in the area, which might explain the ongoing pain and sensitivity during urination.

Another consideration is the possibility of a condition known as urethral syndrome, which can cause symptoms similar to a urinary tract infection (UTI) without the presence of an infection. This syndrome can manifest as pain during urination, increased sensitivity, and difficulty fully emptying the bladder. Given that you have undergone multiple tests, including cystoscopy, and everything has returned normal, this could be a plausible explanation for your symptoms.

Additionally, psychological factors can also play a significant role in how we perceive pain. If you have developed anxiety or fear surrounding urination due to your past experiences, this could exacerbate your symptoms. The mind-body connection is powerful, and stress or anxiety can lead to increased muscle tension and sensitivity in the pelvic region, further complicating your ability to urinate comfortably.

Regarding the specific incident you mentioned, it is possible that the forceful retraction of the foreskin could have caused some damage to the frenulum or the sensitive tissues around the urethra. If the frenulum is scarred or overly sensitive, it could lead to discomfort during erections or urination. This could also explain why you feel the need to manually assist in urination, as the sensation of blockage could be linked to the sensitivity or discomfort in the area.

To address your concerns, I recommend the following steps:
1. Consult a Urologist: Since you have already seen multiple healthcare providers, it may be beneficial to consult a urologist who specializes in pelvic pain or urethral issues. They may offer insights or treatments that have not yet been explored.

2. Pelvic Floor Therapy: Engaging in pelvic floor physical therapy can help address muscle tension and improve urination mechanics. A trained therapist can provide exercises and techniques to help relax the pelvic floor muscles, which may alleviate some of your symptoms.

3. Psychological Support: If anxiety or fear is contributing to your symptoms, consider speaking with a mental health professional. Cognitive-behavioral therapy (CBT) can be particularly effective in addressing anxiety related to bodily sensations and can help you develop coping strategies.

4. Hydration and Diet: Ensure you are well-hydrated and consider dietary adjustments that may reduce irritation to the bladder and urethra. Avoiding irritants such as caffeine, alcohol, and spicy foods may help.

5. Follow-Up Testing: If symptoms persist, further testing may be warranted to rule out any underlying conditions that may not have been previously identified.

In conclusion, your symptoms are likely multifactorial, involving both physical and possibly psychological components. A comprehensive approach that includes medical evaluation, physical therapy, and psychological support may provide the best outcomes for managing your urethral sensitivity and pain during urination.

First, check for phimosis and excessive foreskin length that may cause infection. You can attempt to retract the foreskin to expose the glans penis: (1) If the glans cannot be exposed, this indicates phimosis. (2) If the glans can be exposed but the foreskin still covers the enti...
