Is laser polypectomy covered by health insurance?
In the past year or two, I have noticed a polyp near my anus (close to the urethral opening) that has been gradually increasing in size.
Recently, I visited a specialized clinic covered by health insurance, where the doctor informed me that it is not hemorrhoids but a polyp.
The bleeding during bowel movements is due to an anal fissure.
However, I do have internal hemorrhoids, likely between stages 2 and 3.
The doctor recommended laser treatment for the internal hemorrhoids, and the polyp could be removed at the same time (which would take about two minutes).
However, the cost would be NT$30,000, and since laser treatment is considered cosmetic, it is not covered by health insurance.
Is this true? If I go to a medical center for laser removal of the polyp, will health insurance cover it? Will I need to be hospitalized?

Dr. Hou Yongji reply Surgery
1.
Currently, laser surgery for hemorrhoids primarily focuses on the treatment of internal hemorrhoids.
However, the use of cutting or coagulation tools during the procedure is considered excessive and only increases medical costs without providing significant benefits.
2.
For inquiries regarding medical centers, please contact the medical center directly for answers.
3.
Please seek treatment from a nearby colorectal surgeon.
Wishing you good health!!!!!

In Taiwan, the coverage of medical procedures under the National Health Insurance (NHI) system can be quite complex, especially when it comes to specific treatments like laser polyp removal. In your case, you mentioned having a polyp near the urethra and internal hemorrhoids, and you were advised to undergo laser treatment for both conditions. However, you were informed that the cost would be around NT$30,000 and that the procedure might not be covered by health insurance due to its classification as a cosmetic procedure.
To clarify, the NHI in Taiwan generally covers medically necessary treatments, but there are specific criteria that must be met for coverage. Laser treatments can sometimes fall into a gray area, particularly if they are perceived as cosmetic rather than medically necessary. For instance, if the polyp is causing significant symptoms such as bleeding or discomfort, it may be classified as a medically necessary procedure, which could potentially qualify for NHI coverage. However, if the procedure is deemed to be primarily for cosmetic reasons, then it would likely not be covered.
When it comes to the question of whether you would need to be hospitalized for the procedure, this can depend on the specific circumstances of your case and the policies of the medical facility. In many cases, procedures like laser polyp removal can be performed on an outpatient basis, meaning you would not need to stay overnight in the hospital. However, if there are complications or if the doctor deems it necessary for your safety, hospitalization may be required.
To ensure that you receive the most accurate information regarding your situation, I recommend the following steps:
1. Consult with a Specialist: Since you have already seen a specialist, it may be beneficial to seek a second opinion from another gastroenterologist or colorectal surgeon who has experience with similar cases. They can provide insight into whether the procedure is medically necessary and if it can be covered by NHI.
2. Check with NHI: Contact the National Health Insurance Administration directly or visit their website to inquire about the specific coverage for laser polyp removal. They can provide detailed information regarding what is covered and under what circumstances.
3. Discuss with the Medical Facility: When you consult with the medical center where you plan to have the procedure, ask them about their policies regarding NHI coverage for laser treatments. They should be able to provide you with information on whether the procedure can be billed to NHI and if any documentation is needed to support your case.
4. Consider Documentation: If your doctor can provide documentation that supports the medical necessity of the procedure, this may help in obtaining coverage from NHI. This could include notes on the symptoms you are experiencing and the potential risks of not having the polyp removed.
In summary, while laser polyp removal may not be covered under certain circumstances, it is essential to gather more information from healthcare professionals and the NHI to determine your options. Being proactive in understanding your rights and the coverage available to you can help alleviate some of the financial burdens associated with medical procedures. Remember, your health is the priority, and seeking the appropriate treatment is crucial.
