Testicular issues
Hello, doctor.
Recently, I have noticed a burning sensation in my urethra when urinating, followed by a feeling of urgency.
The first urination in the morning has an unusual odor, so I took some over-the-counter medication for urethritis, which worked quickly.
I completed a five-day course, but after three days, the same symptoms returned.
After continuing the medication, I experienced immediate improvement, but I started to feel some discomfort in my right testicle.
Although it does not significantly affect my daily life, the discomfort persists.
When I touch a specific spot inside the testicle, I notice increased sensitivity and some pain.
This symptom has not worsened, but it has not improved over the past three days.
If it were prostatitis or urethritis, would these symptoms be present? Of course, I will seek medical attention as soon as possible; I just wanted to assess the situation myself first.

Based on your description, it seems to be caused by urethritis, with a possibility of developing orchitis.
The potential causes could be insufficient fluid intake, holding urine, or frequent sexual activity.
Regardless of the cause, drinking plenty of water and avoiding holding urine will definitely benefit you.
Additionally, it is important to consult a physician for an accurate diagnosis and treatment, rather than frequently using over-the-counter medications, as this may lead to overlooking more serious issues if there are other underlying problems.

Testicular discomfort, especially when accompanied by urinary symptoms such as burning sensation during urination and increased frequency, can be indicative of several underlying conditions. Based on your description, it seems you are experiencing a combination of urinary tract symptoms and discomfort in the right testicle, which may suggest a few possible causes.
1. Urinary Tract Infection (UTI): The burning sensation during urination and the abnormal smell of urine can be signs of a UTI. UTIs can cause irritation in the urinary tract, leading to symptoms like urgency and frequency of urination. If you have been self-medicating with antibiotics for a UTI, it is possible that the initial infection was alleviated, but if the symptoms return, it may indicate that the infection was not completely resolved or that there is a recurrent infection.
2. Urethritis: This is an inflammation of the urethra, often caused by infections, including sexually transmitted infections (STIs). Symptoms can include burning during urination, discharge, and discomfort in the genital area. If you suspect urethritis, it is crucial to get tested for STIs, as they can lead to complications if left untreated.
3. Prostatitis: Inflammation of the prostate gland can also cause urinary symptoms and discomfort in the pelvic region, including the testicles. Prostatitis can be acute or chronic and may present with pain during urination, pelvic pain, and sometimes flu-like symptoms.
4. Epididymitis: This is an inflammation of the epididymis, the tube at the back of the testicle that stores and carries sperm. Epididymitis can cause testicular pain and swelling, and it may be associated with urinary symptoms. It is often caused by infections, including STIs or UTIs.
5. Testicular Torsion or Hernia: Although less likely given your symptoms, testicular torsion (twisting of the spermatic cord) is a medical emergency that presents with sudden severe pain in the testicle. A hernia can also cause discomfort in the groin or testicular area, especially when straining or lifting.
Given that you have noted sensitivity and some pain in the right testicle, it is essential to seek medical attention promptly. A healthcare provider will likely perform a physical examination, possibly including a testicular exam, and may order urine tests, imaging studies, or cultures to determine the exact cause of your symptoms.
In the meantime, it is advisable to avoid self-medicating without proper guidance, as inappropriate use of antibiotics can lead to resistance and complicate diagnosis. Additionally, if you experience any worsening symptoms, such as severe pain, swelling, fever, or nausea, you should seek emergency medical care.
In summary, while conditions like prostatitis or urethritis could explain your symptoms, a thorough evaluation by a healthcare professional is necessary to arrive at an accurate diagnosis and appropriate treatment plan. Early intervention can help prevent complications and ensure better outcomes.
