Regarding white lesions on the inner lining of the mouth?

Doctor: I have developed numerous small white spots along the inner edges of my lips on both sides, and they have been present for three weeks.
They feel slightly noticeable when touched with my fingers, and the sensation is much more pronounced when I touch them with my tongue.
Recently, they have gradually spread, and the inner side of my upper lip has also developed them.
I visited a dermatologist, who only told me it was a bacterial infection.
Can you provide a more specific diagnosis? The ointment prescribed by the doctor doesn't seem to improve the condition; in fact, the spots are increasing in number.
The diameter of the spots is less than 0.5 mm, concentrated near the corners of my mouth.
They are neither painful nor itchy, and so far, there has been no breaking of the skin.
Thank you, and I look forward to your response.

Hello YQ: You have noticed numerous small white spots on the inner edges of your lips.
Many conditions could present with such symptoms.
Based on your description, it is difficult to determine your exact condition.
The most significant characteristic of skin diseases is that "seeing is believing and touching reveals texture." It is indeed challenging to convey a complete picture through text alone.
Therefore, I recommend that when you notice the symptoms described, you should promptly visit a public hospital in your county or city and consult with a dermatologist.
All our physicians are very willing to assist you.

Understanding white bumps or patches inside the mouth can be a concern for many individuals. The presence of white lesions or bumps can arise from various causes, and it is essential to identify the underlying issue to determine the appropriate treatment. Based on the information provided, it seems you are experiencing white spots or bumps on the inner lip, which have persisted for three weeks and are gradually spreading.
Several potential causes could explain the symptoms you are experiencing:
1. Oral Candidiasis (Thrush): This is a fungal infection caused by an overgrowth of Candida yeast. It often presents as white patches that can be scraped off, but in some cases, it may appear as small bumps. Risk factors include a weakened immune system, antibiotic use, or diabetes.

2. Lichen Planus: This is an inflammatory condition that can affect the mucous membranes in the mouth. It may present as white, lacy patches or bumps and can be associated with stress or certain medications.
3. Oral Leukoplakia: This condition involves the formation of white patches in the mouth that cannot be scraped off. It is often associated with irritation from tobacco use, but it can also occur without any obvious cause. While leukoplakia itself is not cancerous, it can sometimes be a precursor to oral cancer, so monitoring is essential.

4. Bacterial Infections: As mentioned by the dermatologist you consulted, bacterial infections can lead to the formation of small white bumps. These may be due to localized infections or irritation from trauma, such as biting the lip or irritation from dental appliances.

5. Allergic Reactions: Allergies to certain foods, dental products, or medications can cause localized reactions in the mouth, leading to the development of white bumps or patches.

6. Mucous Retention Cysts: These are small, painless bumps that can occur due to the blockage of salivary glands. They are usually harmless and may resolve on their own.

Given that your symptoms have persisted for three weeks and are worsening, it is advisable to seek further evaluation from a healthcare professional, preferably an oral specialist or an ear, nose, and throat (ENT) doctor. They can perform a thorough examination and may recommend a biopsy if necessary to rule out more serious conditions.

In terms of treatment, it will depend on the underlying cause. If it is a fungal infection, antifungal medications may be prescribed. For inflammatory conditions like lichen planus, corticosteroids may be used to reduce inflammation. If the issue is related to irritation or trauma, avoiding the irritant and maintaining good oral hygiene can help.

In the meantime, here are some general tips to manage oral health and potentially alleviate your symptoms:
- Maintain Oral Hygiene: Brush your teeth at least twice a day and floss daily to keep your mouth clean.

- Avoid Irritants: Stay away from spicy, acidic, or abrasive foods that may exacerbate irritation.

- Stay Hydrated: Drink plenty of water to keep your mouth moist and help wash away irritants.

- Manage Stress: Since stress can exacerbate oral conditions, consider stress-reduction techniques such as mindfulness or yoga.

In conclusion, while the presence of white bumps in the mouth can be concerning, many causes are benign and treatable. A thorough evaluation by a healthcare professional is essential to determine the exact cause and appropriate treatment plan.
