Freckles
I have undergone laser treatment 5 to 6 times, but the spots have not faded.
Is the combination cream effective? Are there different brands? Should I go to a major hospital to get it from a doctor, or can I just buy it at a pharmacy?

Hello: The cause of freckles is genetic, and they appear after exposure to ultraviolet (UV) radiation.
Treatments can remove existing freckles, but if proper sun protection (UV protection) is not maintained, they will reappear.
Therefore, effective sun protection (applying sunscreen) is crucial.
A sunscreen with a sun protection factor (SPF) of around 30 is recommended, and it should be applied thickly.
Reapplication at noon is advisable, and wearing a hat or using an umbrella to avoid direct sunlight is best.
Sunscreen should also be applied on cloudy days or when staying indoors.
The combination treatment has no substantial effect on freckles and is not recommended.
Sincerely, Dr.
Jiang Yu-Jiao, Dermatologist.

More Info
Freckles, or ephelides, are small, flat, brownish spots that typically appear on sun-exposed areas of the skin, particularly in individuals with lighter skin tones. While they are generally harmless, many people seek treatment for cosmetic reasons. There are several effective treatments for freckles, including laser therapy, topical creams, and professional advice from dermatologists.
Laser Treatments:
Laser therapy is one of the most effective methods for reducing the appearance of freckles. The procedure involves using concentrated beams of light to target and break down the melanin in the skin, which is responsible for the pigmentation of freckles. Common types of lasers used include Q-switched lasers and fractional lasers. Many patients report significant improvement after 5 to 6 sessions, but results can vary based on skin type, the depth of pigmentation, and the specific laser used. If you have undergone multiple sessions without noticeable improvement, it may be worth consulting with your dermatologist to evaluate the treatment plan and consider alternative options.
Topical Creams:
Topical treatments can also be effective in lightening freckles. Creams containing ingredients such as hydroquinone, kojic acid, azelaic acid, or retinoids can help reduce pigmentation over time. The "3-in-1" creams you mentioned may contain a combination of these active ingredients, which can be beneficial. However, the effectiveness of over-the-counter products can vary widely, and prescription-strength formulations may yield better results. It is advisable to consult with a dermatologist to determine the most suitable product for your skin type and condition.
Professional Advice:
When considering treatments for freckles, it is essential to seek professional advice. A dermatologist can assess your skin and recommend the most appropriate treatment options based on your individual needs. They can also provide guidance on the use of topical creams, including the correct application and potential side effects. While some creams may be available over the counter, others may require a prescription, especially those that are more potent.
Brand Considerations:
There are numerous brands that offer creams for hyperpigmentation, including those specifically formulated for freckles. Some well-known brands include Obagi, SkinCeuticals, and Murad. However, the effectiveness of a product can depend on its formulation and your skin type. Therefore, it is best to consult with a dermatologist who can recommend specific brands and formulations that have been clinically proven to be effective.
Conclusion:
In summary, effective treatments for freckles include laser therapy and topical creams. If you have not seen results from laser treatments after several sessions, it may be time to reassess your treatment plan with a dermatologist. Topical creams can also be effective, but their success often depends on the active ingredients and your skin type. Always consult with a healthcare professional before starting any new treatment to ensure it is safe and appropriate for your skin condition.
