Chest tightness in the left side of the heart?
Hello Doctor, I have been feeling a tightness in my left chest every morning upon waking up for the past week.
It is quite uncomfortable, and I often gently press on the area around my heart to alleviate the discomfort, but it persists.
The duration varies, but I feel very uncomfortable for at least two hours after getting up each morning.
I have no prior history of heart disease and would like to ask for your advice on how to improve this situation.
Should I go to the hospital for an examination? Thank you.

Dr. He Dongjin reply Cardiology
Dear,
Young individuals may present with at least two or more of the following symptoms: chest pain, palpitations, fatigue, shortness of breath, dizziness, insomnia, anxiety, panic, and even syncope.
These symptoms are not derived from cardiac issues but are instead caused by excessive sympathetic nervous system activation, leading to a syndrome often referred to as autonomic dysregulation syndrome.
Mitral valve prolapse is merely another manifestation within this syndrome and does not have a causal relationship with the other symptoms.
In some patients, the mitral valve fails to close tightly, resulting in some blood flow regurgitating back into the left atrium during left ventricular contraction, clinically referred to as mitral regurgitation.
The frequency, timing, triggers, and severity of symptoms in patients with mitral valve prolapse can vary significantly from person to person.
Common triggers include: colds, menstrual cycles, emotional stress, lack of sleep, delayed meals, excessive fatigue, or the consumption of stimulants such as tobacco, alcohol, caffeine, and drugs.
It is advisable to consult a cardiologist for evaluation.
If echocardiography confirms mitral valve prolapse, patients should be aware of the conditions that may trigger their symptoms and make efforts to avoid these situations.
Additionally, in conjunction with prescribed medications from their physician, most symptoms can be effectively managed.
Thank you for your letter.
The Changhua Hospital, Department of Health, cares about your well-being.

More Info
When experiencing discomfort in the left chest area, especially if it persists for several days, it is essential to take the symptoms seriously and consider seeking medical attention. Chest discomfort can arise from various causes, some of which may be benign, while others could indicate a more serious underlying condition, such as coronary artery disease (CAD) or other cardiac issues.
Understanding the Symptoms
Your description of feeling a persistent "tightness" or discomfort in the left chest area, particularly upon waking, is noteworthy. While it is common for individuals to experience occasional chest discomfort due to anxiety, muscle strain, or even gastrointestinal issues, the fact that it has been occurring daily for an extended period warrants further investigation.
1. Potential Cardiac Causes:
- Coronary Artery Disease (CAD): This is a condition where the coronary arteries become narrowed or blocked, leading to reduced blood flow to the heart muscle. Symptoms can include chest pain or discomfort, shortness of breath, and fatigue.
- Myocardial Ischemia: This occurs when the heart muscle does not receive enough blood, often due to CAD. It can manifest as chest pain, especially during physical exertion or stress.
- Arrhythmias: Irregular heartbeats can also cause discomfort or a feeling of pressure in the chest.
2. Non-Cardiac Causes:
- Musculoskeletal Issues: Strain or injury to the muscles or ribs can lead to chest discomfort, particularly if you have been engaging in physical activities that may have overexerted your upper body.
- Gastroesophageal Reflux Disease (GERD): Acid reflux can cause a burning sensation in the chest, often mistaken for heart-related pain.
- Anxiety and Stress: Psychological factors can lead to physical symptoms, including chest tightness or discomfort.
When to Seek Medical Attention
Given your symptoms, it is advisable to consult a healthcare provider, especially since the discomfort has been persistent. Here are some guidelines on when to seek care:
- Duration and Frequency: If the discomfort lasts for more than a few minutes or occurs frequently, it is essential to get evaluated.
- Associated Symptoms: If you experience additional symptoms such as shortness of breath, dizziness, nausea, sweating, or pain radiating to the arms, neck, or jaw, seek immediate medical attention.
- Family History: Given your father's history of coronary artery bypass surgery, you may have a higher risk for heart disease, which further emphasizes the need for evaluation.
- Lifestyle Factors: If you smoke, have high blood pressure, diabetes, or high cholesterol, these are additional risk factors that should be considered.
Recommended Actions
1. Schedule a Doctor's Appointment: Discuss your symptoms with a healthcare provider who can perform a thorough evaluation, including a physical examination, and possibly order tests such as an electrocardiogram (ECG), echocardiogram, or stress testing to assess your heart's function.
2. Lifestyle Modifications: While waiting for your appointment, consider making some lifestyle changes:
- Diet: Adopt a heart-healthy diet rich in fruits, vegetables, whole grains, and lean proteins.
- Exercise: Engage in regular physical activity, as tolerated, to improve cardiovascular health.
- Stress Management: Practice relaxation techniques such as deep breathing, meditation, or yoga to help manage anxiety.
3. Monitor Your Symptoms: Keep a diary of your symptoms, noting when they occur, their duration, and any associated activities or stressors. This information can be valuable for your healthcare provider.
In conclusion, while your symptoms may not necessarily indicate a severe condition, it is crucial to take them seriously and seek medical advice. Early evaluation can help identify any potential issues and allow for timely intervention, ensuring your heart health is prioritized.
