Scars after head and neck surgery?
Hello Doctor: I have a tumor in my lateral pharyngeal area that needs to be surgically removed through an incision on my neck.
The doctor mentioned that the incision will be approximately 10 centimeters long (extending from just below the earlobe towards the area near the lower jaw).
Since this is on the neck, it will be quite visible, and I am very concerned about how noticeable the scar will be after the surgery.
Although the doctor said that the incision will follow the natural lines of the skin for better healing and less visibility, I am still quite anxious.
Therefore, I would like to ask the doctor if the scar will really be less noticeable after the surgery? If I request the doctor to use cosmetic sutures during the closure, will that result in less scarring? Additionally, how should I care for the surgical wound to minimize the visibility of the scar? (I know that I can use silicone tape; does that really help?) Thank you for your response, Doctor.

Hello: In response to your question, the appropriate management for a tumor in the lateral neck is surgical excision, unless the tumor is small or superficial.
Generally, it is safer to remove the entire tumor.
Regarding wound closure, if there are no tension-related issues, a subcuticular suture technique can be used.
However, since the neck is a highly mobile area, stronger and more secure sutures are typically employed to prevent wound dehiscence.

Managing scars after neck surgery, especially in a visible area like the neck, can be a significant concern for many patients. Your anxiety about the scar's appearance is completely understandable, given the location and size of the incision. Here are some insights and recommendations based on medical knowledge and practices that can help you manage your scar effectively.
Understanding Scar Formation
Scars form as part of the body’s natural healing process after surgery. The appearance of a scar can be influenced by several factors, including the surgical technique used, the location of the incision, your skin type, and how well you care for the wound post-surgery. In your case, since the incision is approximately 10 cm long and located on the neck, it is crucial to follow proper care protocols to minimize visibility.
Surgical Technique and Scar Appearance
Your surgeon mentioned that the incision would follow the natural lines of your skin, which is a good practice. This technique, known as "Langer's lines," helps in reducing the visibility of scars. Using finer sutures, often referred to as cosmetic or "beauty" sutures, can also help in minimizing scarring. These sutures are designed to be less noticeable and can lead to a more aesthetically pleasing result. If you haven't already, discuss with your surgeon the possibility of using these types of sutures for your procedure.
Post-Operative Care for Scar Management
1. Keep the Wound Clean and Moist: After the initial healing phase, keeping the scar moisturized can help in reducing its appearance. Use a gentle, fragrance-free moisturizer or a silicone gel, which has been shown to be effective in scar management.
2. Avoid Sun Exposure: UV exposure can darken scars and make them more noticeable. Use sunscreen with a high SPF on the scar once it has healed sufficiently (usually after about two weeks) to protect it from sun damage.
3. Silicone Sheets or Gels: Applying silicone sheets or gels can help flatten and soften scars. These products create a protective barrier over the scar and can be particularly effective if used consistently over several weeks or months.
4. Massage the Scar: Once the incision has healed (usually after a few weeks), gentle massage of the scar tissue can help in breaking down the collagen fibers that form the scar, potentially leading to a softer and less noticeable scar.
5. Avoid Smoking: Smoking can impair blood flow and delay healing, which can lead to more pronounced scarring. If you smoke, consider seeking support to quit.
6. Hydration and Nutrition: Staying hydrated and maintaining a balanced diet rich in vitamins and minerals can support the healing process. Vitamins A, C, and E, as well as zinc, play crucial roles in skin health and healing.
7. Follow-Up with Your Surgeon: Regular follow-ups with your surgeon can help monitor the healing process. If you notice any unusual changes, such as increased redness, swelling, or pain, consult your surgeon promptly.
Conclusion
While it is natural to be concerned about the appearance of scars after surgery, following these guidelines can significantly improve the outcome. Remember that every individual heals differently, and factors such as genetics and skin type can influence scar formation. If you have specific concerns or if the scar does not improve over time, don’t hesitate to discuss further options with your surgeon, including potential treatments like laser therapy or surgical revision if necessary.
In summary, while the initial appearance of the scar may cause anxiety, with proper care and attention, you can minimize its visibility and promote optimal healing.
