That's ridiculous?

Hello doctor, I’ve been experiencing some issues lately (I just noticed recently).
I started dating a girl last week, and during our first time together, I had been drinking, smoking, eating poorly, and staying up late.
It seemed like I wasn’t very firm during foreplay, but when it came time to penetrate, I completely lost it.
I’m really concerned, but I attributed it to my recent poor physical condition.

Yesterday, I felt a bit better physically compared to last week.
To be honest, I was very stressed before we were intimate again because I was afraid it would happen like last time (I kept thinking during foreplay if it would be the same again).
However, at least during foreplay, I felt okay (I was trying to hold it together because I was scared).
I thought this time would be fine, but shortly after penetration, I felt strange again and lost it completely.
This has happened twice in one week, and I’m really worried.
I don’t know what the problem is.
Please help me.

Thank you.
My girlfriend even suggested I try with another girl to see if it would be the same, and I feel so sad that I can’t make her happy because this issue is affecting our relationship.

Hello, I think changing the person involved would likely yield the same results.
Your situation, in my opinion, is caused by psychological factors.
The more you focus on it and the more you care, the more counterproductive it may become, potentially worsening the effects.
I suggest you approach this matter with a calm mindset and let things take their natural course; it should gradually improve.

Erectile dysfunction (ED) in young men can be a distressing experience, especially when it occurs unexpectedly. It’s important to understand that ED can stem from a variety of causes, both psychological and physiological. In your case, several factors may be contributing to your difficulties.

Psychological Factors
1. Performance Anxiety: It’s common for young men to experience anxiety about sexual performance, especially if they have had a previous negative experience. This anxiety can create a cycle where the fear of not being able to perform leads to actual difficulties in achieving or maintaining an erection.

2. Stress: Life stressors, whether related to school, work, or personal relationships, can significantly impact sexual function. If you are feeling overwhelmed or anxious about other aspects of your life, this can manifest in sexual performance issues.

3. Relationship Dynamics: The pressure to satisfy a partner can create additional stress. If you feel that your partner's happiness is contingent on your performance, this can exacerbate anxiety and lead to further difficulties.

Physiological Factors
1. Substance Use: Alcohol and nicotine can impair erectile function. Alcohol is a depressant that can reduce sexual arousal and performance, while smoking can affect blood flow, which is crucial for achieving an erection.

2. Lifestyle Choices: Irregular sleep patterns, poor diet, and lack of exercise can all contribute to ED. Sleep is vital for hormonal balance, and inadequate rest can lead to lower testosterone levels, which can affect libido and erectile function.

3. Health Conditions: While less common in younger men, certain medical conditions can lead to ED. These include hormonal imbalances, cardiovascular issues, or neurological conditions. If you have any underlying health concerns, it’s essential to address them with a healthcare provider.

Solutions and Recommendations
1. Consult a Healthcare Provider: Since you’ve already experienced ED on multiple occasions, it’s advisable to consult a healthcare professional. They can conduct a thorough evaluation, including a physical examination and possibly blood tests, to rule out any underlying medical issues.

2. Address Psychological Factors: Consider speaking with a therapist or counselor who specializes in sexual health. They can help you work through performance anxiety and other psychological barriers.

3. Lifestyle Modifications: Focus on improving your overall health. This includes:
- Regular Exercise: Aim for at least 150 minutes of moderate aerobic activity each week. Exercise can improve blood flow and reduce stress.

- Healthy Diet: Incorporate a balanced diet rich in fruits, vegetables, whole grains, and lean proteins. Avoid excessive alcohol and quit smoking if you can.

- Sleep Hygiene: Establish a regular sleep schedule and create a restful environment to improve your sleep quality.

4. Communication with Your Partner: Openly discussing your concerns with your girlfriend can alleviate some pressure. A supportive partner can help reduce anxiety and create a more relaxed atmosphere.

5. Gradual Exposure: If performance anxiety is a significant issue, consider taking a step back from penetrative sex. Focus on intimacy and other forms of sexual expression that do not involve penetration, which can help reduce pressure and build confidence.

Conclusion
Erectile dysfunction in young men is often multifactorial, involving a combination of psychological and physiological factors. By addressing lifestyle choices, seeking professional help, and maintaining open communication with your partner, you can work towards overcoming these challenges. Remember, you are not alone in this experience, and many young men face similar issues. With the right approach and support, improvement is certainly possible.
