Frequent urination issue 2

Hello Dr.
Hsu,
I previously asked you about my frequent urination issue.
The urine test conducted by the doctor showed no problems, and my urine was quite diluted.
(Can early-stage cystitis be detected through such tests?) However, the color of my nighttime urine and the urine I produce in the morning is quite yellow.
I also experienced some urinary retention recently, often sitting in a chair.
I subsequently underwent an ultrasound and X-ray examination, and the doctor did not mention any signs of kidney stones; he only noted that I had a bit of excess stool and prescribed magnesium oxide tablets and Bladderon for me.
Now, I no longer feel discomfort while breathing, but I still experience frequent urination, approximately every 20 minutes (the urine volume is moderate, not very little; I believe this may be related to the Bladderon medication).
Could you please help me understand what factors might be causing this issue? It has been troubling me for a long time.
Thank you.

kk, 20~29 year old female. Ask Date: 2006/04/16

Dr. Xu Weikai reply Urology

KK: Hello, the phenomenon of frequent urination can be caused by many factors.
Stress, anxiety, infections, and increased fluid intake are all contributing factors to frequent urination.
Recommendation: It is advisable to keep a record of your urine output each time.
If the volume is consistently greater than 150 milliliters, there should not be a significant concern, and it is likely due to increased fluid intake.
However, if it is consistently less than 50 milliliters, further examination is necessary.
This is a response from Dr.
Hsu Wei-Kai, Director of the Urology Department at Taitung Hospital, Ministry of Health and Welfare.

More Info

Frequent urination, or polyuria, can be a distressing symptom that significantly impacts daily life. Understanding the underlying causes and exploring potential solutions is essential for relief. Based on your description, it seems that you have been experiencing persistent urinary issues, including frequent urination, changes in urine color, and a history of urinary retention.

Possible Causes of Frequent Urination
1. Infection: Urinary tract infections (UTIs) are a common cause of frequent urination. Even if initial tests show no signs of infection, it is possible for a UTI to be present, especially if symptoms persist. In some cases, the infection may be in the bladder or kidneys, which may not always be detected in a standard urine test.

2. Bladder Irritation: Conditions such as interstitial cystitis or bladder inflammation can lead to increased urinary frequency. This irritation can be caused by various factors, including dietary choices (caffeine, alcohol, spicy foods), stress, or certain medications.

3. Neurological Conditions: Issues with nerve signals that control the bladder can lead to frequent urination. Conditions like multiple sclerosis or diabetes can affect bladder function.

4. Medications: Certain medications, including diuretics or bladder relaxants like the one you mentioned (Bladderon), can increase urination frequency. It’s important to discuss with your physician whether your current medications may be contributing to your symptoms.

5. Prostate Issues: In men, prostate enlargement can lead to urinary frequency and urgency. Though you did not mention any specific prostate symptoms, it’s worth considering if you are male.

6. Psychological Factors: Anxiety and stress can also manifest as frequent urination. If you find yourself worrying about needing to use the restroom, this could exacerbate the frequency.

Solutions and Management Strategies
1. Further Testing: Since your symptoms have persisted despite initial evaluations, consider asking your healthcare provider for further testing. This may include a urine culture to check for hidden infections, bladder function tests, or imaging studies to rule out any anatomical issues.

2. Medication Review: Discuss your current medications with your doctor. If Bladderon or any other medication is contributing to your symptoms, your doctor may adjust your treatment plan.

3. Lifestyle Modifications:
- Fluid Management: Monitor your fluid intake, especially before bedtime or outings. Reducing caffeine and alcohol can also help.

- Bladder Training: Gradually increasing the time between bathroom visits can help train your bladder to hold more urine.

- Dietary Changes: Keeping a food diary to identify any potential irritants can be beneficial.

4. Stress Management: If anxiety is a contributing factor, consider techniques such as mindfulness, meditation, or cognitive-behavioral therapy (CBT) to help manage your stress levels.

5. Follow-Up Care: Regular follow-ups with your healthcare provider are crucial. If your symptoms do not improve, or if they worsen, further evaluation may be necessary.

Conclusion
Frequent urination can stem from a variety of causes, and it’s important to approach the issue holistically. By working closely with your healthcare provider, you can identify the underlying factors contributing to your symptoms and develop a tailored treatment plan. Remember, persistent symptoms warrant further investigation, and you should not hesitate to seek additional opinions or referrals to specialists if necessary. Your comfort and quality of life are paramount, and addressing these urinary issues is an important step towards achieving that.
