Testicular injury

Third inquiry: Due to a severe impact to the testicular area from a careless classmate, it has been a month and a half, and I have not fully recovered.
I have seen three doctors and undergone an ultrasound examination.
The doctors all said it is likely a contusion from the impact.
I have taken anti-inflammatory and pain relief medication for two weeks.
Now I notice that the painful area seems to be above the left testicle, and I am unsure if it is the epididymis.
There is a small lump about the size of one-fifth of the testicle that is not fixed in place, and that area feels painful and uncomfortable.
After the injury, the left testicle does not react like the right one; it shrinks immediately when exposed to cold, as if it is experiencing elastic fatigue, and it takes longer to respond.
The injured left testicle often hangs lower than the right one, which was not the case before.
I use hot water for warm compresses while showering, but the effect has not been very good.
It has been a month and a half, and I still feel discomfort, which is concerning.
I am unsure if this is a residual effect.
Please provide me with an explanation, doctor.
Thank you.

Hello: Scrotal injuries can be classified into blunt trauma, penetrating injuries, burns, bites, and skin avulsions of the scrotum.
In cases of scrotal injury, it is essential to assess whether there is a testicular rupture to increase the likelihood of testicular preservation and to reduce complications such as infection and ischemic atrophy.
During a physical examination, the presence of bruising and swelling in the scrotum, along with dark purple discoloration, indicates severe contusion or trauma to the testicles or scrotum.
A scrotal ultrasound can be performed to evaluate for testicular rupture.
Management depends on the type and severity of the injury.
If a testicular rupture is identified, immediate surgical intervention is typically required to repair the damaged testicle.
Based on your description, a scrotal ultrasound can be arranged for further comparison.
Sincerely, Dr.

More Info

Testicular injuries, particularly those resulting from blunt trauma, can be quite concerning and may lead to a variety of symptoms and complications. Based on your description, it seems that you have been experiencing persistent discomfort and changes in the appearance and function of your left testicle following a significant impact. Here, I will address your concerns regarding symptoms, recovery, and potential long-term effects.

Symptoms of Testicular Injury
After a blunt trauma to the testicular area, common symptoms may include:
1. Pain and Discomfort: This is often the most immediate symptom following an injury. Pain can vary in intensity and may be localized to the injured area or radiate to other regions.

2. Swelling and Bruising: The affected testicle may appear swollen or bruised due to internal bleeding or inflammation.

3. Changes in Position: As you noted, the injured testicle may hang lower than the uninjured one. This can occur due to swelling or changes in the supporting structures.

4. Sensitivity to Temperature: The testicles are sensitive to temperature changes. If one testicle is not responding normally to cold (as you described), it could indicate an underlying issue.

5. Presence of a Mass: The small, movable mass you described above the left testicle could be the epididymis (the structure that stores and matures sperm) or a hematoma (a localized collection of blood outside of blood vessels).

Recovery Process
Recovery from a testicular injury can vary based on the severity of the trauma. In cases of minor contusions, symptoms may resolve within a few weeks with conservative treatment, including:
- Rest: Avoiding activities that may exacerbate the injury.

- Pain Management: Over-the-counter pain relievers or prescribed medications can help manage discomfort.

- Ice Therapy: Applying ice packs to the area can reduce swelling and pain.

In your case, since you have been experiencing discomfort for over a month, it may be indicative of a more significant injury, such as a hematoma or even a potential injury to the testicle itself.

Concerns and Long-term Effects
1. Epididymitis: If the pain persists, it could be due to inflammation of the epididymis (epididymitis), which can occur after trauma. This condition may require antibiotics if an infection is suspected.

2. Testicular Atrophy: In some cases, if the blood supply to the testicle is compromised, it may lead to atrophy (shrinkage) of the affected testicle.

3. Fertility Issues: Although many individuals recover fully from testicular injuries, there is a potential risk for fertility issues, especially if the injury was severe or if there was damage to the testicular tissue.

4. Psychological Impact: Persistent pain and changes in the testicular area can lead to anxiety and concerns about sexual health and fertility.

When to Seek Further Medical Attention
Given that you have been experiencing discomfort for a month and have noted changes in the appearance and function of your left testicle, it is advisable to follow up with a urologist. They may recommend further imaging studies, such as a Doppler ultrasound, to assess blood flow to the testicle and rule out complications like torsion or significant hematoma.

In conclusion, while many testicular injuries can heal with conservative management, persistent symptoms warrant further evaluation to ensure there are no underlying complications. It is essential to address these concerns promptly to avoid potential long-term effects on your health and well-being.
