White bumps on the glans?

Hello Dr.
Chen, I recently noticed a white bump on my glans while showering.
It's about one centimeter and slightly raised, but it doesn't hurt or itch.
A few days ago, I visited a doctor who diagnosed it as a bacterial infection and prescribed a bottle of topical medication, along with Erythromycin tablets, Acetaminophen 500 mg, and Pancreatin capsules 250 mg.
I've been applying the medication and taking the pills as directed, but the white spot is still there.
Could you please tell me what this might be? Is it a cause for concern? I wrote to you a few days ago but didn't receive a response, so I'm writing again.
Thank you!

Zhi, 20~29 year old female. Ask Date: 2006/05/10

Hello: There is a white lump on the glans, but I'm not sure where it is located on the glans.
If it is located at the coronal sulcus, it could be a pearly penile papule.
Other possibilities include infections (bacterial or fungal), warts, etc.
It is best to see a urologist or dermatologist for evaluation.
Sincerely, Dr.
Chen Hsiu-Yi, Urology.

More Info

Understanding white bumps on the glans can be concerning, especially when they appear suddenly and without pain. Based on your description, it seems you have noticed white, slightly raised bumps on the glans of your penis, which your doctor has attributed to a bacterial infection. Let's break down the potential causes, treatments, and what you should consider moving forward.

Possible Causes
1. Pearly Penile Papules: These are small, harmless bumps that can appear around the glans. They are a normal anatomical variant and are not caused by any infection or disease. They typically do not require treatment.

2. Fordyce Spots: These are small, painless, pale bumps that can appear on the glans or shaft of the penis. They are sebaceous glands that are not associated with any disease and are completely benign.

3. Bacterial Infection: Since your doctor mentioned a bacterial infection, it is possible that the bumps are related to an infection such as balanitis, which is inflammation of the glans. This can occur due to poor hygiene, irritation, or an imbalance of bacteria.

4. Fungal Infection: Conditions like candidiasis (yeast infection) can also cause white patches or bumps on the glans. This is more common in individuals with diabetes or those who are immunocompromised.

5. Sexually Transmitted Infections (STIs): Certain STIs can present with bumps or lesions on the genital area. If you have had unprotected sex, it is important to consider this as a possibility.

Treatment Options
Your doctor has prescribed several medications, including:
- Topical Antiseptic (妥膚淨): This is likely intended to reduce bacterial load on the skin and promote healing.

- Antibiotics (汎妥黴素膠囊): These are prescribed to treat any underlying bacterial infection.

- Pain Relief (愛舒疼錠): This is likely to help with any discomfort, although you mentioned that you do not have pain.

It is crucial to follow your doctor's instructions regarding the use of these medications. If you have been using them as directed and have not seen improvement, it may be time to follow up with your healthcare provider.

When to Seek Further Medical Attention
1. Persistence of Symptoms: If the white bumps do not improve or worsen after completing your prescribed course of treatment, you should return to your doctor. They may need to reassess your condition or consider alternative diagnoses.

2. New Symptoms: If you develop additional symptoms such as pain, itching, discharge, or fever, seek medical attention promptly.

3. Concerns About STIs: If you have had unprotected sexual contact, it is wise to get tested for STIs, even if you do not have other symptoms.

Conclusion
While the presence of white bumps on the glans can be alarming, many causes are benign and treatable. It is essential to maintain good hygiene and follow your doctor's treatment plan. If symptoms persist or worsen, do not hesitate to seek further medical advice. Remember, open communication with your healthcare provider is key to addressing your concerns effectively.
