Genital itching
Hello doctor, I experienced itching in the genital area six months ago, and recently I have been having symptoms of itching and pain, along with a milky white discharge.
What could this condition be? What are the possible causes? Are there any preventive measures? What foods should I avoid or that may be beneficial? I notice that the itching occurs after sexual intercourse, but we use condoms, so I'm unsure where the problem lies.
Please help clarify this for me.

Dr. Zhong Wenzhen reply Obstetrics and Gynecology
Hello Ms.
Jj: The primary source of infections in the female perineal area (including vaginitis) comes from gastrointestinal bacteria originating from the anus, such as Escherichia coli and Candida species.
Infections caused by Candida can lead to the formation of a white, curd-like discharge and cause itching in the perineal area.
Beverages containing Lactobacillus may help improve the gut microbiota and could be beneficial.
Wishing you a speedy recovery!

Itchy genital symptoms can be quite uncomfortable and concerning, and they can arise from various causes. Based on your description of experiencing itching, pain, and a white discharge, it is essential to consider several potential conditions that could be responsible for these symptoms.
One common cause of itchy genital symptoms, especially with a white discharge, is a yeast infection, also known as candidiasis. This condition is caused by an overgrowth of the fungus Candida, which is normally present in small amounts in the body. Factors that can contribute to a yeast infection include antibiotic use, hormonal changes (such as those occurring during menstruation), a weakened immune system, and certain lifestyle factors like wearing tight clothing or not maintaining proper hygiene.
Another possibility is bacterial vaginosis (BV), which is caused by an imbalance of the normal bacteria in the vagina. BV can lead to symptoms such as itching, a fishy odor, and a grayish-white discharge. It is important to note that BV is not a sexually transmitted infection, but sexual activity can influence its occurrence.
Sexually transmitted infections (STIs) such as trichomoniasis, chlamydia, or gonorrhea can also cause similar symptoms, including itching, pain, and abnormal discharge. While you mentioned using condoms during sexual activity, it is still possible to contract certain STIs, as condoms do not provide 100% protection against all infections.
To prevent these conditions, consider the following strategies:
1. Maintain Good Hygiene: Regularly wash the genital area with mild soap and water, and avoid using scented products that can irritate the skin.
2. Wear Breathable Clothing: Opt for cotton underwear and avoid tight-fitting clothes to reduce moisture buildup, which can promote fungal growth.
3. Limit Antibiotic Use: Only take antibiotics when prescribed by a healthcare provider, as they can disrupt the natural balance of bacteria and lead to yeast infections.
4. Manage Stress and Diet: Stress can weaken the immune system, making you more susceptible to infections. A balanced diet rich in probiotics (found in yogurt and fermented foods) can help maintain a healthy vaginal flora.
5. Avoid Irritants: Steer clear of douches, scented tampons, and other products that can irritate the vaginal area.
If you are experiencing persistent symptoms, it is crucial to consult a healthcare professional for a proper diagnosis and treatment plan. They may perform a pelvic exam and take samples for laboratory testing to determine the exact cause of your symptoms. Treatment options may include antifungal medications for yeast infections, antibiotics for bacterial infections, or specific treatments for STIs.
In summary, while itchy genital symptoms can be caused by various factors, identifying the underlying cause is essential for effective treatment and prevention. Regular check-ups with a healthcare provider can help monitor your sexual health and address any concerns promptly.
