Swollen?

Hello, doctor! The area below both of my ears feels hard and swollen when I touch it.
It hurts when I press on it! It also hurts when I open my mouth too wide or chew something hard.
What could this be?

Dr. Ye Dawei reply Otolaryngology

Sir, it may be an infection.
You can try taking antibiotics.

Understanding ear and throat pain, particularly when accompanied by swelling below the ears, can be quite complex. Based on your description, it sounds like you may be experiencing symptoms related to the salivary glands, specifically the parotid glands, which are located just in front of the ears. Swelling and tenderness in this area can indicate several potential issues, including infections, inflammation, or even dental problems.

One common cause of swelling below the ears is sialadenitis, which is an infection of the salivary glands. This condition can occur when the ducts of the glands become blocked, often due to dehydration, reduced saliva flow, or the presence of stones (sialolithiasis). Symptoms typically include swelling, pain, and sometimes fever. The pain may worsen when you eat or think about food, as this stimulates saliva production.

Another possibility is mumps, a viral infection that can cause swelling of the parotid glands. Although mumps has become less common due to vaccination, it can still occur, especially in unvaccinated individuals. Symptoms include fever, headache, and swelling of the salivary glands.

Additionally, your symptoms could be related to temporomandibular joint (TMJ) disorders. The TMJ is located just in front of the ear, and issues with this joint can lead to pain that radiates to the ear and surrounding areas. Symptoms may include jaw pain, difficulty chewing, and clicking or popping sounds when moving the jaw.

Given that you also experience pain when opening your mouth wide or chewing hard foods, it’s possible that there could be an underlying dental issue, such as a cavity or an abscessed tooth, particularly if the pain is localized to one side. Dental infections can cause referred pain to the ear and jaw area.

To address your concerns, I recommend the following steps:
1. Consult a Healthcare Professional: It’s essential to see an ear, nose, and throat (ENT) specialist or a dentist for a thorough examination. They can assess the swelling, check for signs of infection, and determine if there are any dental issues contributing to your symptoms.

2. Hydration and Oral Hygiene: Ensure you are well-hydrated, as dehydration can exacerbate salivary gland issues. Maintain good oral hygiene to prevent infections.

3. Warm Compresses: Applying a warm compress to the swollen area may help alleviate discomfort and promote drainage if there is an infection.

4. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage pain and reduce inflammation.

5. Avoid Hard Foods: Until you have a clearer understanding of the cause of your pain, it may be wise to avoid hard or chewy foods that could exacerbate your symptoms.

In summary, the swelling and pain you are experiencing below your ears could be due to a variety of causes, including salivary gland infections, TMJ disorders, or dental issues. A thorough evaluation by a healthcare professional is crucial for an accurate diagnosis and appropriate treatment.
