Concussion

Hello Doctor: At the end of last month, I was in a car accident and hit my head, losing consciousness at the scene.
My friends tried to wake me for a long time, and it was only when the person responsible for the accident picked me up that I slowly regained consciousness.
When I arrived at the hospital, the doctor noticed that my head was completely swollen and ordered an X-ray.
Later, the doctor diagnosed me with a concussion and kept me under observation for 2 hours.
Within 24 hours, I returned to the hospital due to dizziness, headaches, and vomiting.
Now, a month later, I have noticed that I sometimes experience headaches, dizziness, numbness in my hands, overall weakness, difficulty concentrating, muscle soreness throughout my body, fatigue, and occasionally I feel blank.
Sometimes, I also experience dizziness accompanied by a darkening of my vision.
I am unsure if these are aftereffects of the concussion or something else.
What should I be aware of?

Xiao Ke, 10~19 year old female. Ask Date: 2006/05/28

Dr. Ding Xianwei reply Neurosurgery

Hello: The symptoms you mentioned could potentially be sequelae.
However, numbness in the hands and neck pain may indicate a cervical spine injury.
It is still necessary to have a personal consultation for a better diagnosis.
Sincerely, Dr.
Ding Hsien-Wei, Neurosurgery.

More Info

After experiencing a head injury, especially one that involves a concussion, it is crucial to monitor for various symptoms that may indicate ongoing issues or complications. Your situation, having sustained a concussion from a car accident, is concerning, particularly given the symptoms you are currently experiencing.
Concussions are a type of traumatic brain injury (TBI) that can result from a blow to the head or a violent shaking of the head and body. Symptoms can vary widely and may not appear immediately. Common post-concussion symptoms include headaches, dizziness, nausea, confusion, difficulty concentrating, fatigue, and changes in mood. In your case, the symptoms of headaches, dizziness, numbness in your hands, weakness, difficulty concentrating, muscle aches, fatigue, and episodes of feeling "spaced out" are all significant and warrant further evaluation.

What to Watch For After a Head Injury:
1. Persistent Headaches: While headaches are common after a concussion, if they persist or worsen, it could indicate complications such as post-concussion syndrome or other serious conditions.

2. Dizziness and Balance Issues: Ongoing dizziness can affect your ability to perform daily activities and may indicate vestibular dysfunction, which is common after a concussion.

3. Neurological Symptoms: Numbness, tingling, or weakness in the limbs can suggest nerve involvement or other neurological issues that need immediate attention.

4. Cognitive Difficulties: Trouble concentrating, memory issues, or confusion can be signs of post-concussion syndrome, which can last for weeks or even months after the initial injury.

5. Fatigue and Sleep Disturbances: Increased fatigue and changes in sleep patterns are common after a concussion and can exacerbate other symptoms.

6. Mood Changes: Emotional instability, irritability, or depression can occur following a concussion and may require psychological support.

7. Visual Disturbances: Blurred vision or seeing "black spots" can indicate issues with the brain's processing of visual information.

Recommendations for Follow-Up:
Given the symptoms you are experiencing, it is essential to follow up with a healthcare provider, preferably a neurologist or a specialist in brain injuries. Here are some steps you should consider:
- Neurological Evaluation: A thorough neurological examination can help assess your symptoms and determine if further imaging (like an MRI) is necessary to rule out any complications such as bleeding or swelling in the brain.

- Cognitive Testing: Neuropsychological testing can help evaluate your cognitive function and identify specific areas of difficulty.

- Vestibular Rehabilitation: If dizziness is a significant issue, vestibular therapy may help improve balance and reduce symptoms.

- Physical Therapy: If you are experiencing muscle weakness or coordination issues, physical therapy can assist in regaining strength and function.

- Monitoring Symptoms: Keep a detailed log of your symptoms, noting when they occur, their intensity, and any triggers. This information can be valuable for your healthcare provider.

- Rest and Recovery: Ensure you are allowing adequate time for recovery. Avoid activities that could risk another head injury, and consider limiting screen time and cognitive strain until you feel better.

- Support System: Engage with family and friends for support, as emotional and psychological well-being is crucial during recovery.

In summary, your symptoms are concerning and should not be ignored. It is essential to seek further medical evaluation to ensure that you receive appropriate care and support for your recovery. Remember, recovery from a concussion can take time, and it is important to be patient with yourself as you heal.
