Strange discharge from the nipple?
Hello, Dr.
Hsieh: The nipples do not hurt and there are no lumps, but I can see some white substances with the naked eye.
Out of curiosity, I squeezed it and found that I could express something resembling whiteheads.
Is this a normal condition? I initially thought it would resolve after a few days, but it seems to have reappeared! Please advise me on how to handle this.
Thank you.

Dr. Xie Wenhuai reply Breast and Thyroid
A young woman with white discharge from her nipples may be experiencing secretion from the mammary ducts or sebaceous gland secretion.
As long as she keeps the area dry and avoids scratching, it should not pose any issues.

Understanding unusual nipple discharge can be concerning, especially when it presents as a white substance that resembles a whitehead or comedone. While it is essential to approach this issue with caution, it is also important to understand that not all nipple discharges are indicative of a serious condition.
In your case, the discharge you describe could be related to several benign conditions. One common cause is a condition known as mammary duct ectasia, where a milk duct beneath the nipple dilates, fills with fluid, and thickens, leading to a sticky discharge. This condition is often seen in women who are approaching or have gone through menopause, but it can occur at any age. Another possibility is a condition called periductal mastitis, which is an inflammation of the ducts beneath the nipple. This can lead to discharge and sometimes pain or tenderness, although you mentioned that you do not experience pain or lumps.
It is also worth considering that hormonal changes can affect breast tissue and lead to discharge. For example, during menstrual cycles, hormonal fluctuations can cause changes in breast tissue that may result in discharge. If you are not experiencing any pain, lumps, or other concerning symptoms, it may not be an immediate cause for alarm. However, the fact that the discharge recurs after a few days suggests that it is worth monitoring.
To manage this situation, here are some recommendations:
1. Maintain Hygiene: Ensure that the area is kept clean and dry. Gently wash the nipple area with mild soap and water to prevent any potential irritation or infection.
2. Avoid Squeezing: While it may be tempting to squeeze the nipple to see if more discharge comes out, this can irritate the ducts and potentially lead to inflammation or infection.
3. Monitor Symptoms: Keep track of any changes in the discharge, such as color, consistency, or if it becomes associated with pain, swelling, or any other unusual symptoms.
4. Consult a Healthcare Provider: Since the discharge is recurring, it is advisable to consult a healthcare provider, preferably a specialist in breast health or a gynecologist. They may perform a physical examination and, if necessary, imaging studies like a mammogram or ultrasound to rule out any underlying issues.
5. Consider Hormonal Evaluation: If you suspect that hormonal fluctuations may be contributing to the discharge, discussing this with your healthcare provider could be beneficial. They may recommend tests to evaluate your hormone levels.
In summary, while the presence of a white discharge from the nipple can be benign, it is essential to monitor the situation and consult a healthcare professional for a thorough evaluation. Early intervention can help address any potential issues and provide peace of mind. Remember, your health is paramount, and seeking professional advice is always a prudent step when it comes to unusual bodily changes.
