Gastrointestinal issues
Hello Dr.
Yuan, I hope you are doing well.
I would like to ask you a few questions amidst your busy schedule.
For the past month, I have been experiencing diarrhea and have visited the hospital multiple times.
Initially, the doctor suggested it might be gastroenteritis and prescribed medication for it, along with a bland diet.
However, my condition does not seem to improve; the diarrhea fluctuates, sometimes I have two days of normal bowel movements, and then on the third day, I have diarrhea again.
The doctor then conducted a stool test, which came back normal with no signs of inflammation or bleeding.
Occasionally, I feel discomfort in my lower abdomen, so the doctor changed my medication to Gut Comfort and Gas-X.
In the past three days, my bowel movements have been more regular, but after taking the medication, I sometimes feel a sense of fullness in my lower abdomen, which is uncomfortable.
Occasionally, my bowel movements start off normal but then become somewhat loose (though not significantly so).
Would this still be considered diarrhea? The doctor mentioned that if I do not have diarrhea this week, I do not need to return for a follow-up.
Could you please advise me on what might be the issue, as I have never experienced this before? Thank you for taking the time to read my lengthy message!

Hello: I would like to know if you have experienced any blood in your stool, weight loss, fever, anemia, or a family history of colorectal cancer.
If none of these apply, it is more likely that you are experiencing irritable bowel syndrome following gastroenteritis.
However, other conditions such as specific infections, inflammatory bowel disease, or even colorectal tumors cannot be ruled out.
I recommend a light diet and temporarily avoiding milk.
If you gradually improve, that would be fine; otherwise, further examination is necessary.
Sincerely, Dr.
Yuan Hongdao, Internal Medicine.

More Info
It sounds like you've been experiencing a challenging and uncomfortable situation with your gastrointestinal health. Based on the information you've provided, it seems that you have been dealing with persistent diarrhea and abdominal discomfort for about a month, despite various treatments and dietary adjustments. Let's break down your situation and explore some potential underlying issues and recommendations.
Understanding Your Symptoms
1. Persistent Diarrhea: The fact that you have been experiencing diarrhea intermittently, with periods of normal bowel movements followed by loose stools, suggests that there may be an underlying issue affecting your gastrointestinal tract. While your stool tests have come back normal, indicating no inflammation or blood, it’s important to consider other factors that could contribute to your symptoms.
2. Abdominal Discomfort: The sensation of fullness or discomfort in your lower abdomen, especially after taking medication, could be related to several factors. Medications like肚痊寧 (which is likely an antidiarrheal) and 加斯克兒 (which is likely an antacid or digestive aid) can sometimes lead to gastrointestinal side effects, including bloating or discomfort.
3. Dietary Factors: You mentioned that you have been following a light diet, which is generally advisable during episodes of gastrointestinal distress. However, certain foods, even if they seem light, can still trigger symptoms in sensitive individuals. Foods high in fat, dairy, or certain fibers can sometimes exacerbate diarrhea or discomfort.
Possible Underlying Issues
1. Irritable Bowel Syndrome (IBS): Given the pattern of your symptoms, one possibility is that you may be experiencing IBS, a common functional gastrointestinal disorder characterized by symptoms like abdominal pain, bloating, and changes in bowel habits (diarrhea and/or constipation). IBS can be triggered by stress, dietary choices, or changes in gut flora.
2. Food Intolerances: Another consideration is the possibility of food intolerances or sensitivities, such as lactose intolerance or gluten sensitivity. These conditions can lead to gastrointestinal symptoms similar to what you are experiencing.
3. Infection or Inflammation: Although your stool tests were normal, it’s still possible that a mild infection or inflammation could be present, especially if you have had any recent travel or dietary changes. Sometimes, infections can resolve on their own but leave lingering symptoms.
Recommendations
1. Monitor Your Symptoms: Keep a detailed diary of your symptoms, including what you eat, your bowel movements, and any other relevant factors (like stress levels). This can help identify potential triggers and patterns.
2. Dietary Adjustments: Consider trying an elimination diet to identify any food intolerances. Start by removing common triggers such as dairy, gluten, and high-fat foods, and then gradually reintroduce them one at a time to see if symptoms worsen.
3. Hydration and Electrolytes: Ensure you are staying well-hydrated, especially if you are experiencing diarrhea. Consider electrolyte solutions if diarrhea persists, as they can help replenish lost fluids and minerals.
4. Follow-Up with Your Doctor: If your symptoms do not improve or worsen, it’s essential to follow up with your healthcare provider. They may recommend further testing, such as imaging studies or referrals to a gastroenterologist for more specialized care.
5. Consider Stress Management: Since stress can significantly impact gastrointestinal health, incorporating stress-reducing techniques such as mindfulness, yoga, or therapy may be beneficial.
In conclusion, while your symptoms are concerning, they are not uncommon, and with careful monitoring and adjustments, you may find relief. It’s crucial to maintain open communication with your healthcare provider and advocate for further evaluation if necessary. Your health and comfort are paramount, and addressing these gastrointestinal issues can significantly improve your quality of life.
