Premature Ejaculation: Sensitivity After Long Abstinence - Urology

Suspected premature ejaculation?

Hello doctor, I feel that whenever I go a long time without sexual intercourse with a female, I become very sensitive and even suspect I might have premature ejaculation.
If I haven't engaged in sexual intercourse for a year, after having physical contact with a female, just inserting my penis into the vagina (without thrusting) makes me unable to hold back ejaculation.
However, before penetration, I don't have any thoughts of ejaculating.
It seems that I need to maintain a consistent frequency of intercourse for about a month to gradually achieve longer duration, which can eventually last for 10 to over 20 minutes.
I would like to understand if this qualifies as premature ejaculation.
Thank you.

Premature ejaculation (PE) is a common sexual dysfunction characterized by the inability to control ejaculation, leading to ejaculation occurring sooner than desired, either before or shortly after penetration. Your experience of heightened sensitivity after a prolonged period of abstinence is not uncommon and can be influenced by several factors, including psychological, physiological, and situational aspects.

When you mention that after a long period of abstinence, you find yourself ejaculating almost immediately upon penetration, this could be attributed to several factors. Firstly, sexual arousal can build up significantly during periods of abstinence, leading to increased sensitivity and a heightened response to sexual stimuli. This heightened sensitivity can make it more challenging to control ejaculation when sexual activity resumes.

In terms of defining whether this constitutes premature ejaculation, the key factors to consider are the timing of ejaculation in relation to sexual activity and the level of distress it causes. The Diagnostic and Statistical Manual of Mental Disorders (DSM-5) defines PE as a persistent or recurrent pattern of ejaculation occurring during partnered sexual activity within one minute following vaginal penetration, and this pattern must cause significant distress or interpersonal difficulty. Based on your description, if you are able to achieve a longer duration of intercourse (10-20 minutes) after a period of regular sexual activity, it suggests that your body may be adapting to the sexual stimulus over time.

It's also important to note that sexual performance can vary greatly from one individual to another and can be influenced by factors such as anxiety, stress, and overall physical health. The psychological aspect is particularly significant; anxiety about performance can exacerbate the feeling of urgency to ejaculate. Therefore, if you are feeling anxious about your sexual performance, this could contribute to the sensation of premature ejaculation.

If you are concerned about this issue, it may be beneficial to explore some strategies to help manage sensitivity and improve control over ejaculation. Techniques such as the "start-stop" method, where you pause stimulation before ejaculation, or the "squeeze" technique, where pressure is applied to the base of the penis to reduce arousal, can be effective. Additionally, practicing mindfulness and relaxation techniques can help reduce anxiety and improve sexual performance.

Consulting with a healthcare professional, such as a urologist or a sexual health specialist, can provide you with tailored advice and potential treatment options. There are also medications available that can help delay ejaculation, and therapy can address any underlying psychological factors contributing to the issue.

In summary, while your experience may align with characteristics of premature ejaculation, it is essential to consider the context and the impact it has on your sexual relationships and personal well-being. Engaging in open communication with your partner and seeking professional guidance can help you navigate this concern effectively.
