Questions about varicocele?
Hello Dr.
Hsu,
I apologize for bothering you with more questions.
A few days ago, I went for a follow-up appointment at Chang Gung Hospital.
I initially thought I would need to have my stitches removed, but it seems that I don’t need to.
The wound appears to be sealed with adhesive tape directly.
I had only been changing the outer tape daily, and I originally thought there were stitches underneath.
Will just using tape to cover the wound cause it to heal more slowly? How many days after the surgery can I remove the tape? The tape has been on for over a week without being changed, and it’s starting to feel a bit sticky.
Additionally, there seems to be slight swelling.
How long will the soreness, numbness, and tenderness around the wound take to show significant improvement? At least how many weeks should I wait before I can start exercising? What are the limits? Should I stop if I feel pain? I would greatly appreciate your answers.
Thank you very much!

Xiao Ming: Nice to see you again.
Regarding wound management, some doctors prefer to use traditional sutures, as they feel more secure and are more accustomed to them, although the downside is that they require removal.
Other doctors prefer to adopt newer methods, and generally, there is not much difference in wound healing between the two approaches.
When using subcuticular sutures, adhesive strips are typically applied to reinforce the tension on the wound.
Generally, after about a week, you can remove the strips to inspect the wound, lightly disinfect it, and then apply new strips (at this point, it is recommended to use cosmetic tape, which can be applied continuously for 6 months to ensure the wound heals smoothly and beautifully, almost making you forget that you had surgery).
Pain and numbness around the wound, if there are no other complications such as infections, usually improve gradually over about 3 months.
If the wound is healing well, you might consider applying warm compresses to accelerate the healing process.
The method is simple: use a warm towel to cover the wound for about 5-10 minutes, three times a day, for several days, which should lead to significant improvement.
Note: If the wound becomes noticeably painful during warm compresses or if any other discomfort occurs, stop immediately.
Since the wound is relatively small and not located on a major muscle group, I believe that full recovery of original activity should occur within 6 weeks at the latest.
Initially, there will be pain when resuming activity, but as long as it is tolerable, there should not be too many restrictions.
Note: Just to answer your potential question in advance: after varicocele surgery, the enlarged veins will gradually disappear about 6 months post-operation.
This is a response from Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

Hello! Thank you for reaching out with your questions regarding post-operative care and recovery after varicocele surgery. It's completely normal to have concerns about your recovery process, and I'm here to provide you with some guidance based on medical knowledge.
Firstly, regarding the use of adhesive strips (often referred to as surgical tape) instead of stitches, this is a common practice in many surgical procedures. The adhesive strips are designed to hold the skin edges together while allowing for healing. They can be effective in promoting healing without the need for traditional sutures. As for your concern about the healing process, the adhesive strips should be left in place until they start to peel off on their own, which typically occurs around 7 to 10 days post-surgery. If the tape remains firmly attached and is not causing irritation, it’s generally okay to leave it on longer. However, if you notice any signs of infection (such as increased redness, swelling, or discharge), you should contact your healthcare provider.
In terms of the slight swelling and discomfort you’re experiencing, it’s not uncommon to have some residual swelling and tenderness in the area after surgery. This can be due to inflammation as your body heals. The sensation of numbness or tingling can also occur as nerves recover from the surgical procedure. Typically, you can expect significant improvement in these symptoms within a few weeks, but complete resolution may take longer, depending on individual healing rates.
As for resuming physical activity, it’s crucial to follow your surgeon’s specific recommendations. Generally, light activities can often be resumed within a week or two, but more strenuous activities, especially those that involve heavy lifting or straining, should be avoided for at least 4 to 6 weeks. The key is to listen to your body; if you experience pain during an activity, it’s a sign to stop and rest. Gradually increase your activity level as tolerated, and always consult your doctor if you’re unsure.
In summary, here are some key points to keep in mind for your post-operative care:
1. Adhesive Strips: Leave them on until they start to peel off naturally. If they remain firmly attached after a week and are not causing irritation, it’s generally fine to keep them on longer.
2. Swelling and Discomfort: Mild swelling and discomfort are normal. Monitor for signs of infection and consult your doctor if you have concerns.
3. Resuming Activities: Light activities can often be resumed within a week or two, but avoid strenuous activities for at least 4 to 6 weeks. Listen to your body and stop if you experience pain.
4. Follow-Up Care: Keep all follow-up appointments with your healthcare provider to monitor your recovery and address any concerns.
If you have any further questions or if your symptoms worsen, don’t hesitate to reach out to your healthcare provider for personalized advice. Wishing you a smooth recovery!
