Pain during urination after ejaculation?

Hello doctor, recently after ejaculation, I immediately feel the urge to urinate.
At the moment of urination, there is a sharp pain in the penis that lasts for about 3 to 4 seconds.
Rather than describing it as pain in the penis, it feels more like pain in the glans.
I have a history of excessive masturbation, and since this issue started, I have reduced the frequency of masturbation.
Although I have sought medical attention, the problem remains unresolved.
Every time I ejaculate, urination is painful.
What could be the cause of this? Thank you.

You may have issues related to prostate or urinary tract infections, and it is recommended that you seek evaluation and treatment from a urologist.

More Info

Post-ejaculation pain, especially during urination, can be a distressing symptom and may arise from various underlying causes. Given your description, it seems that the pain is localized to the glans (the head of the penis) and occurs immediately after ejaculation when you attempt to urinate. This discomfort lasting for a few seconds can be attributed to several factors, which I will outline below.

Possible Causes
1. Urethritis: Inflammation of the urethra, often caused by infections (bacterial or viral), can lead to pain during urination. If you have had unprotected sexual encounters, sexually transmitted infections (STIs) like gonorrhea or chlamydia could be potential culprits.

2. Prostatitis: Inflammation of the prostate gland can cause pain during urination and ejaculation. Chronic prostatitis may not always present with obvious symptoms, but it can lead to discomfort during urination or ejaculation.

3. Pelvic Floor Dysfunction: Tension or dysfunction in the pelvic floor muscles can lead to pain during urination and ejaculation. This is often associated with overactivity or tightness in these muscles, which can be exacerbated by frequent masturbation or sexual activity.

4. Post-ejaculatory Pain Syndrome: This condition is characterized by pain after ejaculation, which can sometimes be accompanied by pain during urination. The exact cause is not always clear, but it may involve a combination of psychological and physiological factors.

5. Nerve Irritation: Overstimulation of the nerves in the pelvic region due to excessive masturbation can lead to temporary pain or discomfort. This might explain why you noticed a decrease in your urge to masturbate after the onset of pain.

Recommendations for Management
1. Medical Evaluation: Since you have already sought medical attention without resolution, it may be beneficial to revisit your healthcare provider or seek a urologist's opinion. They may recommend tests such as urinalysis, STI screening, or imaging studies to rule out infections or anatomical issues.

2. Hydration: Ensure you are well-hydrated, as concentrated urine can irritate the urethra and exacerbate pain.

3. Avoid Irritants: Reduce or eliminate the use of products that may irritate the genital area, such as scented soaps or lotions.

4. Pelvic Floor Exercises: If pelvic floor dysfunction is suspected, pelvic floor physical therapy may be beneficial. A trained therapist can guide you through exercises to relax and strengthen the pelvic floor muscles.

5. Medication: Depending on the underlying cause, medications such as anti-inflammatory drugs or antibiotics (if an infection is present) may be prescribed. If prostatitis is diagnosed, specific treatments targeting the prostate may be necessary.

6. Behavioral Modifications: Reducing the frequency of masturbation and allowing time for recovery may help alleviate symptoms.
7. Follow-Up: Persistent symptoms warrant follow-up care. If initial treatments do not provide relief, further evaluation by a specialist may be necessary.

Conclusion
Post-ejaculation pain during urination can stem from various causes, including infections, inflammation, or pelvic floor issues. A thorough evaluation by a healthcare professional is crucial to determine the underlying cause and appropriate treatment. In the meantime, consider lifestyle modifications and follow-up care to manage your symptoms effectively. If you experience worsening symptoms or additional concerns, do not hesitate to seek medical attention promptly.
