Second abortion

Hello, doctor.
I am 30 years old and had an abortion two years ago.
Recently, due to high stress, my ovulation has been irregular, which has led to another pregnancy.
Since I have immigrated overseas and my financial situation is very unstable, I cannot keep this child.
However, I am extremely afraid that having another abortion will affect my future fertility and cause complications such as infertility.
As far as I know, there is only surgical abortion available here, and the use of RU486 is not widely promoted.
I am really scared of undergoing another dilation and curettage (D&C) procedure, fearing it might harm my uterus.
Doctor, am I being overly worried?

S, 30~39 year old female. Ask Date: 2006/07/17

Dr. Xu Junzheng reply Obstetrics and Gynecology

The likelihood of experiencing complications from surgical abortion and RU486 is the same, so there is no need for concern.
Thank you!

It's understandable to have concerns about the implications of a second abortion on your future fertility and overall health, especially given your previous experience and current circumstances. Let's break down your concerns and provide some clarity based on medical knowledge.

Firstly, it's important to acknowledge that having an abortion, particularly a surgical one, can carry risks. However, when performed by a qualified healthcare provider in a safe environment, the risks of complications are generally low. Most women who have had one or even multiple abortions go on to have healthy pregnancies in the future. The key factors that influence future fertility include the method of abortion, the presence of any complications, and individual health factors.

In your case, since you mentioned that your first abortion was two years ago and you are now facing another pregnancy that you feel unable to continue, it's crucial to consider the following:
1. Method of Abortion: Surgical abortions, such as dilation and curettage (D&C), are common and generally safe when performed by experienced professionals. The risk of complications, such as uterine perforation or scarring (Asherman's syndrome), is low but can occur. Medical abortions, using medications like mifepristone (RU486), are also safe and effective, and they do not require surgical intervention. If you are concerned about surgical methods, discussing medical abortion options with your healthcare provider may be beneficial.

2. Impact on Future Fertility: Research indicates that having one or two abortions does not significantly affect a woman's ability to conceive in the future. The majority of women who have had abortions can conceive and carry a pregnancy to term without complications. However, if there are complications from the abortion, such as infection or uterine scarring, these could potentially impact fertility. It's essential to monitor your health and seek medical advice if you experience any unusual symptoms post-abortion.

3. Psychological Impact: The emotional and psychological effects of abortion can also play a role in your overall well-being and future reproductive health. It's important to address any feelings of anxiety or fear you may have regarding the procedure. Counseling or support groups can provide a safe space to discuss these feelings and help you process your experience.

4. Future Pregnancies: If you decide to proceed with the abortion, it’s advisable to wait for a period before trying to conceive again. This allows your body to heal and can help ensure a healthier pregnancy in the future. Your healthcare provider can give you personalized advice on when it might be best to try for another pregnancy.

5. Consultation with a Healthcare Provider: Given your concerns about future fertility and the potential risks associated with another abortion, it would be wise to consult with a healthcare provider. They can provide you with detailed information about the procedures available, assess your individual health status, and help you make an informed decision that aligns with your circumstances.

In conclusion, while it's natural to have fears about the implications of a second abortion, many women successfully conceive and have healthy pregnancies after one or more abortions. The key is to ensure that you receive care from qualified professionals and to take care of your physical and emotional health throughout the process. If you have further questions or need support, don't hesitate to reach out to a healthcare provider who can guide you through your options and help alleviate your concerns.
