Ear and Throat Issues: Symptoms of Ear Infections and Nasopharyngeal Cancer - Otolaryngology

Question!

I have recently noticed that my ears are often moist and have an unpleasant odor, and the earwax has turned grayish-black.
Could this be middle ear infection? Additionally, does nasopharyngeal cancer have symptoms? (My father passed away from nasopharyngeal cancer, and it was already in the advanced stage when discovered!) I have a habit of smoking and have had nasal issues since childhood.
I have seen a doctor who mentioned a slight deviation of the nasal septum and the presence of nasal polyps.
Please help clarify these concerns.
Thank you!

Dr. Jian Huangqi reply Otolaryngology

It could be otitis media or otomycosis.
Nasopharyngeal carcinoma may present with tinnitus, diplopia, nasal discharge or phlegm with blood, and in advanced stages, there may be cervical lymphadenopathy, dizziness, and headaches.
You should consult an otolaryngologist.
Thank you.

More Info

The symptoms you are experiencing, such as a moist and foul-smelling ear, along with changes in earwax color, could indeed indicate an ear infection, specifically otitis media, which is an infection of the middle ear. This condition can lead to the accumulation of fluid behind the eardrum, which may become infected, resulting in a range of symptoms including pain, pressure, and sometimes a foul odor due to bacterial growth. The presence of gray-black earwax may suggest the presence of debris or infection, and it is essential to have this evaluated by a healthcare professional, preferably an ear, nose, and throat (ENT) specialist.

Regarding your concerns about nasopharyngeal cancer, it is understandable to be vigilant, especially given your family history. Nasopharyngeal cancer can present with various symptoms, including:
1. Nasal Obstruction or Congestion: This can manifest as difficulty breathing through the nose or a feeling of fullness.

2. Nasal Bleeding: Frequent nosebleeds can be a sign, although they are not exclusive to cancer.

3. Ear Symptoms: These may include hearing loss, a feeling of fullness in the ear, or ear infections, which can occur due to blockage of the Eustachian tube.

4. Throat Symptoms: This can include a persistent sore throat, difficulty swallowing, or a sensation of a lump in the throat.

5. Swollen Lymph Nodes: Particularly in the neck, swollen lymph nodes can indicate that cancer has spread.

6. Unexplained Weight Loss: This can occur with many types of cancer, including nasopharyngeal cancer.

Given your smoking history and existing nasal issues, it is crucial to monitor your symptoms closely. Smoking is a known risk factor for various cancers, including those of the head and neck. The presence of a deviated septum and nasal polyps can also contribute to chronic nasal problems, which may increase the risk of infections and other complications.

If you are experiencing persistent symptoms or any new symptoms, it is advisable to seek medical attention. An ENT specialist can perform a thorough examination, which may include imaging studies or a biopsy if necessary, to rule out malignancy. Regular follow-ups are essential, especially considering your family history and personal risk factors.

In summary, your symptoms could indicate an ear infection, and it is vital to have them evaluated by a healthcare professional. Additionally, being aware of the symptoms of nasopharyngeal cancer is crucial, especially given your background. Early detection and intervention can significantly improve outcomes, so do not hesitate to seek medical advice if you have concerns.
