My preauricular sinus?
Doctor, I would like to ask if you recommend surgery for my condition.
Since I was young, I have had a persistent discharge of white purulent fluid from my preauricular sinus, and I am the only one in my family with this issue.
It also has a foul odor, and I experience itching about five times a week, accompanied by pus discharge.
Sometimes the fluid from the right preauricular sinus smells, and at other times it switches to the left side.
The odor is so strong that I have to wash it with soapy water to get rid of it.
There was a period when I suddenly had no itching or odor, but I experienced some pain, so I ignored it.
However, the area around the sinus became slightly red and swollen, and I noticed several dried yellow pus spots on my ear.
I was alarmed and went to the hospital, where the doctor extracted a lot of foul-smelling yellow pus.
They prescribed me an ointment but did not mention surgery or any preventive care.
I am still confused.
Now, my condition has returned to the previous state with white pus and odor.
Could you please advise me on the following: 1.
Do you recommend surgery? 2.
How can I prevent the opening from smelling or itching? Thank you.

Dr. Hu Kexin reply Otolaryngology
Hello: If the preauricular sinus is not acutely inflamed and is causing no significant disruption to daily life, surgery is generally considered.
However, it is still important to have your attending physician evaluate your condition.
Sincerely, Dr.
Hsu, Otolaryngology.

Understanding Preauricular Sinus: Symptoms, Treatment, and Surgery Options
Preauricular sinus, also known as preauricular pit or preauricular cyst, is a congenital condition characterized by a small opening or pit located near the front of the ear, often at the junction of the ear and the face. This condition can lead to recurrent infections, which may present as discharge, foul odor, and discomfort, as you have described in your situation.
1. Symptoms: The symptoms of a preauricular sinus can vary from person to person. Commonly, individuals may experience:
- Discharge of pus or fluid from the sinus, which can be white or yellow and may have a foul odor.
- Itching or irritation around the area.
- Redness and swelling if an infection occurs.
- Pain during flare-ups, especially if the sinus becomes inflamed or infected.
Given your description of recurrent infections, foul-smelling discharge, and the need for medical intervention to clean out the pus, it is clear that your preauricular sinus is symptomatic and causing you significant discomfort.
2. Treatment Options: The management of a preauricular sinus primarily depends on the frequency and severity of the symptoms. Here are the common approaches:
- Conservative Management: For mild cases, regular cleaning of the area and topical antibiotics may be sufficient to manage infections. However, this approach often does not resolve the underlying issue and may lead to recurrent problems.
- Surgical Intervention: Given your recurrent symptoms and the impact on your quality of life, surgical intervention is often recommended. The surgery typically involves excising the entire sinus tract to prevent future infections. This procedure is usually performed under local or general anesthesia, depending on the complexity and the patient's comfort level.
- Postoperative Care: After surgery, it is essential to follow your surgeon's instructions for care to minimize the risk of infection and promote healing. This may include keeping the area clean and dry, using prescribed topical antibiotics, and avoiding activities that could irritate the area.
3. Preventing Odor and Itching: To manage the odor and itching associated with a preauricular sinus, consider the following:
- Maintain good hygiene by cleaning the area gently with mild soap and water.
- Avoid using harsh soaps or products that may irritate the skin.
- If you notice any signs of infection, such as increased redness, swelling, or discharge, seek medical attention promptly to prevent complications.
In conclusion, based on your symptoms and the recurrent nature of your condition, I would recommend discussing surgical options with your healthcare provider. Surgery can provide a long-term solution by removing the sinus tract and preventing future infections. Additionally, maintaining good hygiene and seeking prompt treatment for any signs of infection can help manage your symptoms in the meantime. Always consult with a qualified healthcare professional to tailor the treatment plan to your specific needs and circumstances.
