Abnormal bleeding

Dear Doctor: I have been experiencing some light bleeding about 1 to 2 weeks after my menstrual period for approximately six months (I am unsure if this is ovulation bleeding; I had similar episodes several years ago that resolved on their own).
However, starting last month, I have had continuous bleeding for two months after my period, which comes and goes.
Sometimes it consists of only streaks of deep brown blood, and at other times, there are small clots (similar in color to pork liver).
The amount can be similar to the flow I experience 3 to 4 days after my period.
This bleeding continues until my actual menstrual period begins.
I usually experience some abdominal pain and lower back discomfort when my period starts (especially this month, my back pain has been so severe that I can hardly stand up).
I am using these symptoms to determine if my period has truly started, but I am concerned if this is normal.
Thank you.

Hello: Ovulation bleeding typically lasts about 3 days and does not continue until the next menstrual period.
Please come in for an examination to determine if there are any issues.

Abnormal bleeding in women can be a source of significant concern and confusion, especially when it deviates from the expected menstrual cycle. Your description of experiencing bleeding 1-2 weeks after your menstrual period, along with the presence of brownish discharge and occasional small clots, raises several important considerations.

Firstly, it is essential to understand that abnormal bleeding can arise from various causes, including hormonal imbalances, structural abnormalities in the reproductive system, and even lifestyle factors. In your case, the timing of the bleeding suggests it could be related to ovulation. Ovulation bleeding, or mid-cycle spotting, is relatively common and occurs due to the hormonal fluctuations that happen around the time of ovulation. This type of bleeding is usually light and may be accompanied by other symptoms such as mild cramping.

However, the fact that you have been experiencing this bleeding consistently for several months, and that it has increased in duration and intensity, warrants further investigation. It is crucial to rule out other potential causes of abnormal bleeding, such as:
1. Hormonal Imbalances: Conditions like polycystic ovary syndrome (PCOS) or thyroid disorders can lead to irregular menstrual cycles and abnormal bleeding patterns. Hormonal fluctuations can cause the endometrial lining to shed irregularly, leading to unexpected bleeding.

2. Uterine Abnormalities: Structural issues such as fibroids, polyps, or endometrial hyperplasia can also cause abnormal bleeding. These conditions may lead to heavier bleeding or prolonged bleeding episodes.

3. Infections: Pelvic inflammatory disease (PID) or sexually transmitted infections (STIs) can cause abnormal bleeding and should be considered, especially if there are accompanying symptoms like pelvic pain or unusual discharge.

4. Stress and Lifestyle Factors: High levels of stress, significant weight changes, or changes in physical activity can impact your menstrual cycle and lead to irregular bleeding. You mentioned that you have been under stress, which could be a contributing factor.

5. Medications: Certain medications, including hormonal contraceptives, can cause breakthrough bleeding. If you are on any form of hormonal treatment, it is worth discussing with your healthcare provider.

Given your symptoms, it is advisable to consult a healthcare professional for a thorough evaluation. This may include a pelvic exam, ultrasound, and possibly blood tests to assess hormone levels. Keeping a detailed record of your menstrual cycle, including the timing, duration, and characteristics of any bleeding, can provide valuable information for your healthcare provider.

In the meantime, if you experience severe pain, heavy bleeding, or any other concerning symptoms, seek medical attention promptly. It is essential to address any underlying issues to ensure your overall reproductive health and well-being.

In summary, while mid-cycle bleeding can be a normal occurrence for some women, the persistence and changes in your bleeding pattern suggest that further evaluation is necessary. Understanding the underlying causes of abnormal bleeding is crucial for effective management and peace of mind.
