Suspected tear of the foreskin?
Hello, doctor.
I recently started dating a girlfriend, and last Sunday (the 7th), we had vigorous intercourse.
There were no unusual symptoms at that time.
A few days later, after I masturbated on Thursday (four days later), I suddenly noticed a red ulcer on the upper part of the foreskin (where it retracts).
The ulcer is oriented horizontally (when looking down) and does not cause significant pain.
Upon inspection, there is no bleeding or obvious wound, but it is continuously oozing a clear fluid.
Could this be a sexually transmitted infection? If not, can I apply the prescription cream (Pikning) that a previous urologist gave me? Or what over-the-counter medication should I purchase? Thank you!
A-Kai, 30~39 year old female. Ask Date: 2006/08/14
Dr. Xu Weikai reply Urology
A-Kai: Hello, the most common symptoms of infection include redness, swelling, skin breakdown, ulcers, and pain.
Generally, during the acute phase, there may be some discharge, which can sometimes have an unpleasant odor.
As for sexually transmitted infections? I cannot determine that.
I believe there are two possibilities: 1.
Infection, such as bacterial or fungal infections; 2.
Allergy: allergic reactions to condoms or female secretions.
Treatment will depend on the type; for infections, a topical antibiotic ointment is necessary, while for allergies, a corticosteroid ointment should be used.
This is a response from Dr.

More Info
It sounds like you're experiencing some concerning symptoms related to your frenulum, which is the sensitive tissue that connects the foreskin to the glans of the penis. Based on your description, it seems you may have sustained a minor injury or irritation, possibly due to vigorous sexual activity or masturbation. The symptoms you're experiencing, such as redness, ulceration, and the discharge of a clear fluid, could indicate a few different things, including irritation, infection, or even a mild allergic reaction.
Symptoms of Frenulum Tears or Irritation
1. Redness and Swelling: This is often the first sign of irritation or injury. The area may appear inflamed.
2. Ulceration or Lesions: The presence of a sore or ulcer can indicate a more significant injury or infection.
3. Discharge: Clear fluid can be a sign of irritation or an inflammatory response. If the fluid were pus or accompanied by a foul odor, it could suggest an infection.
4. Pain or Discomfort: While you mentioned that there is no significant pain, any discomfort in the area should be monitored.
Possible Causes
1. Frenulum Tear: This can occur during vigorous sexual activity or masturbation, leading to minor injuries that may not bleed but can become inflamed.
2. Infection: Conditions such as sexually transmitted infections (STIs) can cause similar symptoms. However, the absence of pain and bleeding makes a severe infection less likely.
3. Allergic Reaction: Sometimes, irritation can arise from products such as lubricants, soaps, or even latex condoms.
Treatment Options
1. Topical Treatments: If you have a prescription for a topical ointment (like the "皮克寧" ointment you mentioned), it may help soothe the irritation. Always follow your doctor's instructions regarding its use.
2. Over-the-Counter Products: You might consider using a mild antiseptic or a soothing cream designed for sensitive skin. Avoid any products that contain alcohol or strong fragrances, as these can further irritate the area.
3. Hygiene: Maintain good hygiene by gently cleaning the area with mild soap and water. Avoid any harsh soaps or scrubs.
4. Avoiding Irritation: Refrain from sexual activity until the area has healed to prevent further irritation or injury.
5. Monitoring Symptoms: Keep an eye on the symptoms. If they worsen or if you develop additional symptoms (such as fever, increased pain, or a change in the nature of the discharge), seek medical attention promptly.
When to Seek Medical Attention
- If the symptoms do not improve within a few days.
- If you experience increased pain, swelling, or discharge that changes in color or odor.
- If you develop systemic symptoms like fever or chills.
Conclusion
While it is possible that you may have a minor injury or irritation, it is essential to monitor your symptoms closely. If you have concerns about sexually transmitted infections, consider getting tested, as many STIs can present with similar symptoms. Consulting with a healthcare provider, especially a urologist, would be the best course of action to ensure proper diagnosis and treatment. They can provide you with a thorough examination and guide you on the appropriate treatment options based on your specific situation.
