Areola issues

There are many small white spots on the areola (similar to milia, but not exactly).
What could this be? Previously, there was itching and peeling, and yesterday I noticed the appearance of many flat white spots.
What condition could this be?

Linzi, 20~29 year old female. Ask Date: 2006/08/15

Dr. Yang Mingqi reply Breast and Thyroid

Hello, based on your description, these white spots may be glands that are characteristic of the areola and are not indicative of any disease.
However, I still recommend that you consult a breast surgeon or dermatologist for confirmation.

Understanding the appearance of white spots on the areola can be concerning, and it’s important to approach this issue with a clear understanding of potential causes and treatments. The areola, the pigmented area surrounding the nipple, can exhibit various changes due to a range of factors, including hormonal fluctuations, skin conditions, and infections.

One common cause of white spots on the areola is the presence of Montgomery glands, which are sebaceous glands located in the areola. These glands can become more prominent during hormonal changes, such as those occurring during pregnancy or menstruation. They typically appear as small, white or flesh-colored bumps and are completely normal.
Another potential cause could be eczema or dermatitis, which can lead to itching, peeling, and the formation of white spots. This condition can be triggered by irritants, allergens, or even hormonal changes. If the skin around the areola is dry, red, or inflamed, it may indicate a dermatological issue that requires treatment.

Folliculitis, an inflammation of the hair follicles, can also present as small white spots. This condition can occur due to bacterial or fungal infections, and it may be accompanied by itching or discomfort.
In some cases, fungal infections such as candidiasis can lead to white spots on the skin, including the areola. This is more common in individuals with compromised immune systems or those who are taking antibiotics, which can disrupt the natural balance of flora on the skin.

If the white spots are accompanied by symptoms such as severe itching, pain, or discharge, it is crucial to seek medical attention. A healthcare provider can perform a physical examination and may recommend tests to determine the underlying cause.
Treatment options will vary depending on the diagnosis. For instance, if the white spots are due to eczema, topical corticosteroids or moisturizers may be prescribed to alleviate symptoms. If a fungal infection is diagnosed, antifungal creams or medications may be necessary.
In conclusion, while white spots on the areola can be benign, it is essential to monitor any accompanying symptoms and consult a healthcare professional for an accurate diagnosis and appropriate treatment. Self-diagnosing can lead to unnecessary anxiety, so professional guidance is always recommended when it comes to changes in skin appearance, especially in sensitive areas.
