Regarding varicocele?
Hello, Dr.
Hsu.
I started experiencing occasional discomfort and a feeling of heaviness in both testicles during last summer.
In October of last year, I went to Kaohsiung Medical University for an examination.
The doctor initially suspected a varicocele.
Later, I underwent a scrotal ultrasound, and the doctor stated that there were no abnormal findings.
A physical examination also revealed no new symptoms.
Although the discomfort has been intermittent, it hasn't been severe enough to cause me much concern.
Recently, the left side feels fine, but the right side seems to be more prone to discomfort.
What can I do to improve this situation? Thank you for your response.

Dr. Xu Weikai reply Urology
Hello, local application of warm compresses for about 5 minutes each time, three times a day, should help improve the condition.
If the pain worsens after the compress, it may indicate a concurrent infection, and you should immediately stop the warm compresses and seek antibiotic treatment.
This is a response from Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

More Info
Varicocele, a condition characterized by the enlargement of veins within the scrotum, can often lead to discomfort and various symptoms, including a feeling of heaviness or aching in the testicles. Understanding the causes, symptoms, and management strategies for varicocele is essential for those experiencing related issues.
Causes of Varicocele
Varicocele occurs when the valves in the veins that carry blood away from the testicles fail to function properly. This malfunction can lead to blood pooling in the veins, causing them to enlarge. The exact cause of this condition is not always clear, but it is believed to be related to anatomical differences in the venous drainage of the testicles. Varicocele is more common on the left side due to the angle at which the left testicular vein drains into the renal vein, making it more susceptible to increased pressure.
Symptoms of Varicocele
Many individuals with varicocele may not experience significant symptoms. However, common symptoms include:
1. Dull Aching Pain: This is often described as a feeling of heaviness or discomfort in the scrotum, particularly after prolonged standing or physical activity.
2. Visible Enlarged Veins: Some may notice swollen or enlarged veins in the scrotum, which can resemble a "bag of worms."
3. Testicular Atrophy: In some cases, varicocele can lead to a reduction in testicle size.
4. Fertility Issues: Varicocele has been associated with male infertility, as it can affect sperm production and quality.
Management Tips
If you are experiencing discomfort due to varicocele, here are some management strategies that may help alleviate symptoms:
1. Supportive Underwear: Wearing supportive underwear or a jockstrap can help reduce discomfort by providing support to the scrotum and minimizing movement.
2. Avoid Prolonged Standing: If possible, try to avoid standing for long periods, as this can exacerbate symptoms. Taking breaks to sit or lie down may provide relief.
3. Cold Compress: Applying a cold compress to the affected area can help reduce swelling and alleviate pain. Be sure to wrap ice in a cloth to avoid direct contact with the skin.
4. Pain Relief Medications: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage discomfort. However, consult with a healthcare provider before starting any medication.
5. Lifestyle Modifications: Maintaining a healthy weight, engaging in regular exercise, and avoiding heavy lifting can help reduce the strain on the veins in the scrotum.
6. Medical Evaluation: Since you have already undergone an ultrasound and physical examination, it’s essential to continue monitoring your symptoms. If the discomfort persists or worsens, consider consulting a urologist for further evaluation. They may recommend additional imaging studies or discuss potential treatments, including surgical options if necessary.
7. Surgical Intervention: In cases where varicocele causes significant discomfort or fertility issues, surgical options such as varicocelectomy may be considered. This procedure involves tying off the affected veins to redirect blood flow and alleviate symptoms.
Conclusion
While varicocele can be a source of discomfort, many individuals manage their symptoms effectively with lifestyle modifications and supportive measures. It is crucial to stay in communication with your healthcare provider to monitor your condition and explore treatment options if necessary. If you experience any new or worsening symptoms, do not hesitate to seek medical attention.
