Scalp hyperhidrosis
I sweat easily, and it primarily comes from my scalp, especially when wearing a safety helmet or on hotter days, where I sweat profusely.
My hair gets wet several times a day, and since I have very thin hair, my scalp becomes visibly noticeable after sweating, which troubles me greatly.
Can this condition of excessive scalp sweating be treated with surgery?

Dr. Jiang Yujiao reply Dermatology
Hello: In response to your question, sweating is a mechanism used to regulate body temperature, helping to maintain homeostasis.
The amount of sweating is related to factors such as weather conditions, clothing, sweat gland distribution, and emotional stress.
The excessive sweating on your scalp is associated with the ventilation of the head.
After sweating, if the moisture evaporates and there is sufficient airflow, you will feel cooler.
However, if the air temperature is high and there is poor air circulation, sweat will remain in place, leading to a feeling of heat and causing sweat to accumulate like rain.
Some methods to reduce discomfort include (1) thinning your hair, (2) regularly drying sweat with a towel, (3) using a fan to increase air circulation, and (4) relaxing your mind, as there are no effective surgical treatments available.

Excessive scalp sweating, also known as craniofacial hyperhidrosis, can be a distressing condition for many individuals. It is characterized by an overproduction of sweat from the scalp and face, often exacerbated by heat, stress, or physical activity. This condition can significantly impact a person's quality of life, leading to discomfort, embarrassment, and social anxiety.
When it comes to surgical options for treating excessive scalp sweating, there are a few approaches that can be considered. One of the most common surgical interventions is sympathectomy, which involves cutting or clamping the sympathetic nerves that control sweating. This procedure can be performed endoscopically, which is less invasive and typically results in a quicker recovery time compared to traditional open surgery.
However, it's important to note that while sympathectomy can provide relief for many patients, it is not without risks and potential side effects. Some individuals may experience compensatory sweating in other areas of the body, such as the torso or legs, as the body attempts to compensate for the reduced sweating on the scalp. Additionally, there is a risk of surgical complications, such as infection or nerve damage.
Before considering surgery, it is advisable to explore non-surgical treatment options. These may include:
1. Topical Antiperspirants: Prescription-strength antiperspirants containing aluminum chloride can be effective in reducing sweating. These products work by blocking the sweat glands and can be applied directly to the scalp.
2. Botulinum Toxin Injections: Botox injections can temporarily block the nerves that cause sweating. This treatment has been shown to be effective for craniofacial hyperhidrosis and can provide relief for several months before requiring repeat injections.
3. Iontophoresis: This treatment involves using a device that passes a mild electrical current through water and into the skin's surface, which can help reduce sweating in localized areas.
4. Medications: Oral medications, such as anticholinergics, can help reduce sweating by blocking the chemical signals that trigger sweat production. However, these medications may have side effects and are not suitable for everyone.
5. Lifestyle Modifications: Wearing breathable fabrics, using absorbent headbands, and avoiding triggers such as spicy foods or stress can also help manage symptoms.
If you are considering surgical options, it is crucial to consult with a healthcare professional who specializes in hyperhidrosis. They can provide a thorough evaluation, discuss the potential benefits and risks of surgery, and help you determine the best course of action based on your specific situation.
In summary, while surgery can provide relief for excessive scalp sweating, it is essential to weigh the potential benefits against the risks and to consider non-surgical alternatives first. Consulting with a specialist will help you make an informed decision that aligns with your needs and lifestyle.
