Issues related to bowel movements?

Hello Doctor: Recently, I have noticed some abnormalities during bowel movements and would like to consult you: 1.
For about two months, my stools have been irregular in shape (they no longer form the typical cylindrical shape and sometimes resemble diarrhea).
The color is not always yellow-brown; at times, it is dark brown or nearly black.
What could be the issue? (I am aware that my lifestyle is irregular, with late nights and late mornings; could this be related?) 2.
During bowel movements, I often feel gas in my stomach, wanting to burp, and sometimes I experience abdominal bloating and slight pain.
What could this indicate? (I have nasal issues and tend to breathe through my mouth unconsciously while sleeping; could this be related?) I kindly ask for your insights and suggestions on how to improve this situation.
Thank you!

It may be Irritable Bowel Syndrome (IBS): The following relevant information is provided: Clinically, Irritable Bowel Syndrome can present with three main types of symptoms: (1) Chronic intermittent diarrhea, which generally occurs without abdominal pain.
Diarrhea often happens in the morning upon waking or after breakfast, typically resulting in three to four soft stools or stools mixed with some mucus, after which the discomfort usually subsides.
Some individuals may experience thin, pencil-like stools, and this condition may persist for weeks or months before resolving on its own.
(2) Chronic abdominal pain and intermittent constipation, where patients may experience crampy abdominal pain, mostly in the lower left abdomen, which typically resolves after passing gas or having a bowel movement.
Patients often report abdominal bloating as well.
(3) Alternating diarrhea and constipation, where the above two conditions occur in a repetitive cycle.
Treatment: Patients must understand that this condition can only be managed through lifestyle adjustments and medications to alleviate symptoms, but it cannot be cured.
They should learn to adapt and not take it too seriously, as this condition does not lead to serious diseases such as colorectal cancer or inflammatory bowel disease.
For patients with significant symptoms, medications can be used to target and relieve those symptoms.

Based on your description, it seems you are experiencing a combination of irregular bowel movements and abdominal discomfort, which can be concerning. Let's break down your symptoms and explore potential causes and recommendations for improvement.

1. Irregular Stool Shape and Color Changes
The irregular shape of your stools, which you describe as not forming the typical cylindrical shape, can be indicative of several issues. In general, healthy stools should be well-formed and brown in color, which is primarily due to bile pigments. When stools become irregular in shape, such as being more fragmented or resembling diarrhea, it may suggest a few possibilities:
- Dietary Factors: An irregular diet, particularly one low in fiber and high in processed foods, can lead to changes in stool consistency. If your diet lacks sufficient fruits, vegetables, and whole grains, this could contribute to softer, more irregular stools.

- Dehydration: Insufficient fluid intake can lead to harder stools, while excessive fluid can lead to looser stools. It’s essential to maintain adequate hydration, especially if you are consuming a diet low in fiber.

- Gastrointestinal Disorders: Conditions such as Irritable Bowel Syndrome (IBS) or infections can cause changes in bowel habits. IBS, in particular, can lead to alternating patterns of diarrhea and constipation, along with abdominal discomfort.

- Potential Bleeding: The mention of stools appearing dark brown or black can be concerning, as this may indicate the presence of blood, which could be due to bleeding in the upper gastrointestinal tract. This is something that should be evaluated by a healthcare professional.

2. Abdominal Discomfort and Gas
The sensation of gas and bloating, along with the urge to burp, can be linked to several factors:
- Swallowing Air: Mouth breathing during sleep, especially if you have nasal congestion, can lead to swallowing air, which may contribute to bloating and discomfort.

- Dietary Choices: Certain foods, especially those high in carbohydrates (like beans, lentils, and some vegetables), can produce gas during digestion. Additionally, carbonated beverages can introduce extra gas into your system.

- Gastrointestinal Motility: If your digestive system is not moving food through efficiently, it can lead to a buildup of gas and discomfort. This can be influenced by stress, irregular eating patterns, and lack of physical activity.

Recommendations for Improvement
1. Dietary Adjustments: Aim to increase your fiber intake gradually by incorporating more fruits, vegetables, and whole grains into your meals. This can help regulate your bowel movements and improve stool consistency.

2. Hydration: Ensure you are drinking enough water throughout the day. A general guideline is to aim for at least 8 cups (64 ounces) of water daily, but this may vary based on individual needs and activity levels.

3. Regular Eating Schedule: Try to establish a more regular eating pattern. Eating at consistent times can help regulate your digestive system.

4. Physical Activity: Engage in regular physical activity, as this can help stimulate bowel function and reduce bloating.

5. Monitor Symptoms: Keep track of your symptoms, including any changes in stool appearance, frequency, and associated discomfort. If you notice persistent changes, especially if stools are dark or black, or if you experience significant abdominal pain, it’s crucial to consult a healthcare provider for further evaluation.

6. Consider Stress Management: Since stress can impact digestion, consider practices such as mindfulness, yoga, or other relaxation techniques to help manage stress levels.

Conclusion
Your symptoms warrant attention, especially given the changes in stool color and shape. While lifestyle factors may play a significant role, it’s essential to rule out any underlying gastrointestinal conditions. If symptoms persist or worsen, please seek medical advice for a thorough evaluation and appropriate management. Remember, maintaining a healthy lifestyle can significantly impact your digestive health.
