Menstruation

Hello, I experience diarrhea during my menstrual period and have irregular cycles.
I have taken menstrual regulation medication for several years without improvement, so I decided to stop taking it.
My periods are so irregular that I can't even track them.
Could this be due to the uterus practicing ovulation, or could there be another reason (I started menstruating at the age of 9)? Thank you.

Feng, 10~19 year old female. Ask Date: 2006/09/14

Dr. Li Guanghan reply Obstetrics and Gynecology

Menstrual irregularities and digestive issues, such as diarrhea during menstruation, can be interconnected and often stem from a variety of underlying factors. Your experience of irregular periods and gastrointestinal symptoms during your menstrual cycle is not uncommon, and it can be influenced by hormonal fluctuations, lifestyle factors, and even psychological stress.

Firstly, it is important to understand that the menstrual cycle is regulated by hormones, primarily estrogen and progesterone. These hormones can influence not only the reproductive system but also other bodily systems, including the gastrointestinal tract. Many women experience changes in bowel habits during their menstrual cycle due to hormonal shifts. For instance, increased levels of prostaglandins, which are hormone-like substances involved in the regulation of the menstrual cycle, can lead to increased intestinal motility, resulting in diarrhea or loose stools.

In your case, the irregularity of your menstrual cycle could be attributed to several factors:
1. Hormonal Imbalance: Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other endocrine issues can lead to irregular periods. If you have been taking menstrual regulation medications for several years without improvement, it may be worth discussing with your healthcare provider the possibility of underlying hormonal imbalances.

2. Stress and Lifestyle Factors: Stress can significantly impact your menstrual cycle and digestive health. High levels of stress can lead to hormonal fluctuations that may cause irregular periods and gastrointestinal disturbances. Additionally, lifestyle factors such as diet, exercise, and sleep patterns play a crucial role in regulating both menstrual and digestive health.

3. Age and Developmental Factors: Since you mentioned that you started menstruating at the age of 9, it is possible that your body is still adjusting to the hormonal changes associated with menstruation. It is not uncommon for young women to experience irregular cycles during the first few years after menarche as their bodies adapt to the new hormonal environment.

4. Gastrointestinal Conditions: Conditions such as irritable bowel syndrome (IBS) can also cause digestive issues that coincide with your menstrual cycle. IBS is often characterized by abdominal pain, bloating, and changes in bowel habits, which can be exacerbated during menstruation.

Given your symptoms and the lack of improvement with medication, it may be beneficial to seek further evaluation from a healthcare provider. They may recommend blood tests to check hormone levels, imaging studies, or even a referral to a specialist, such as an endocrinologist or a gastroenterologist, depending on your specific symptoms and medical history.

In the meantime, consider keeping a symptom diary to track your menstrual cycle, digestive symptoms, stress levels, and any dietary changes. This information can be invaluable for your healthcare provider in determining the underlying cause of your symptoms and developing an appropriate treatment plan.

Lastly, while it may be tempting to stop taking medications that have not shown improvement, it is essential to consult with your healthcare provider before making any changes to your treatment regimen. They can help you weigh the benefits and risks and explore alternative options that may be more effective for your situation.

In summary, menstrual irregularities and digestive issues can be complex and multifactorial. It is crucial to approach this issue holistically, considering hormonal, lifestyle, and psychological factors, and to seek professional guidance for a comprehensive evaluation and management plan.
